POSITION SUMMARY The Case Management Social Worker performs effective assessments of patients and their families. Based on the needs identified in an assessment, the Social Worker arranges, coordinates, and monitors multiple services from different providers to meet patients needs.

These providers can include physicians, nurses, therapists, dietitians, pharmacists, state and local social service agencies, and community organizations. The incumbent addresses the psychological and social needs of the patients and formulates the discharge plan in collaboration with all appropriate team members. Under the guidance of the Manager of Case Manager, the Social Worker is responsible for conducting and coordinating patient assessments, transition of care, discharge planning affecting patient outcomes, and patient/family education. MINIMUM QUALIFICATIONS AND REQUIREMENTS Licenses and Certifications Required Licensed Social Worker in the state of North Dakota Educational Requirements Bachelors degree in Social Work required Experience Requirements Two years of experience in area of Social Work preferred, particularly in an acute care setting Special Skills or Training Requirements Demonstrated evidence of dependability, diplomacy, and initiative Demonstrated strong communication and interpersonal skills with all levels of internal and external customers, including but not limited to medical staff, patients and families, clinical personnel, other Case Managers and Social Workers, support and technical staff, outside agencies, third party payors, and members of the community Basic understanding of reimbursement issues, denials, and DRGs Personal computer skills using e-mail, word processing, etc.

COMMUNITY BASED SOCIAL WORKER - PATH of Nexus has a new and exciting opportunity in our Community Based Services program for a full-time ND Licensed Social Worker. SUMMARY: The social worker assists eligible children and their caregivers in gaining access to needed medical, social, educational and other services necessary for appropriate care and treatment. Responsibilities DUTIES AND RESPONSIBILITIES(The following list is not comprehensive but highlights the major areas of responsibility for the position.) 1. Provide comprehensive coordination of all tasks required to meet the treatment needs and best interests of service recipients consistent with regulatory agencies (i.e. DHS, COA). a.Comply with local office procedure for referral and service delivery. b.Complete a systemic assessment of the needs of the child and family. c.Establish service parameters with family or legal custodian as child's needs dictate. d.Provide complete and required documentation, within established time lines relating to case management and treatment planning. e.Implement treatment services to meet the needs and goals of the children and families utilizing quality case management practice to provide for the best interest of the child and family. f.Provide direction to other team members when applicable and make recommendations for discharge, finalization and permanency. g.Promote the needs of youth and families to ensure successful outcomes. h.Meet requirements for youth/family contacts. i.Advocate for the development and maintenance of funding and resources to meet youth and family needs at local, state and national levels. j.Develop and maintain community contacts to promote PATH and collaborative efforts. k.Provide crisis intervention as needed to maintain safety and provide for the best interest of the child and family. 2. Conduct job duties from a framework that acknowledges the impact of childhood trauma on children, caregivers, and service providers. Integrates trauma informed care principles into practice by sharing knowledge and understanding of trauma with the child, family and relevant team members. Additionally, utilizes and advocates for the use of trauma-sensitive strategies to support the youth and their family. a.Routinely screens for trauma exposure and related symptoms and demonstrates competence in using screening tool results to enhance service provision. b.Routinely makes resources available and shares knowledge with foster families and providers on trauma exposure, its impact, and trauma-focused treatment. c.Engages in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma. d.Emphasizes continuity of care and collaboration across child-service systems. e.Understand the potential for secondary traumatic stress and practices self-care strategies to mitigate these effects. 3. Meet standards for productivity and caseload size while maintaining appropriate documentation and files to support the services that are provided to families. a.Complete paperwork and compile documentation required by PATH and governing/accrediting agencies; maintain current files on all youth in placement. b.Produce informal and formal reports as requested by parents, referral agencies, courts, etc. 4. Participate in staff and supervision meetings, and required PATH in-service meetings. Understands and supports the PATH and Nexus philosophy and mission. a.Participate in individual and group supervision including clinical consultation to review cases and ensure best practices are provided to all children and families. b.Provide timely and thorough information to supervisor regarding case incidence, service delivery struggles and difficult team processes. c.Utilize supervision options and directives in service delivery. 5. Ensure confidentiality of all treatment and services information pertaining to clients and their foster, adoptive and biological families. 6. Perform other job related responsibilities, tasks and projects as needed.
