Job Requirements

• 2 years' in a Skilled Nursing Community

• Licensed Social Worker

• Excellent knowledge of computer programs including Microsoft Office

• Open and driven communications skills

• Willingness to contribute to our culture

• Superior time management skills

• Prefer experience with discharge planning
