Social Worker

Overview The Social Worker is responsible for the provision of case management and community liaison services in WakeMed Physician Practices- Pediatrics. This includes: assessing safety and social needs of patients during clinic visits; supporting integration of patients with needed medical, social, and developmental services/resources; supporting families of patients in connecting with psychosocial services which directly or indirectly impact the patient's well-being; being a liaison for community-based programs and services, including schools; and, providing direct intervention for social needs while patients are in the practice. It is expected that this individual will participate in hospital and community initiatives that address psychosocial issues. This position will also require maintaining a working knowledge the programs, services, and resources available in the community to support patients and their families.
