STARTING RATE: $23.00 to $26.45 per hour

SHIFT: Full-time, weekdays, 8:00am-4:00pm

JOB DESCRIPTION:

Social Worker to join our team! A Social Worker is responsible to assist the social work department in planning, developing, implementing, and evaluating social services programs and activities for the nursing home residents in accordance with current federal and state regulations. RESPONSIBILITIES:

• Responsible for all required documentation to complete Social Service Functions

• Addresses and documents questions/concerns of residents and family members in a timely manner. Provides all necessary resources for any required services

• Responsible for practicing and protecting all residents' rights

• Provide support, guidance, and therapy to residents to promote greater independence and more meaningful life.

• Maintain communication with necessary departments regarding any updates, changes, or issues concerning the residents.

• Follows nursing home policies and procedures.

• Respects resident's rights and dignity

• Reports any suspected resident abuse as required by government regulations and the nursing home policies.

• Attends all required in-services and seminars.

REQUIREMENTS:

• Bachelor's Degree in Social Work, Human Services, or Psychology with experience in geriatrics.

• Must have one year of supervised social work experience (paid/volunteer) working directly with individuals in a healthcare setting.

• Possess a working knowledge of current federal and state regulations and professional standards.

• Participate in facility surveys conducted by authorized government agencies.

• Maintain a quality working relationship with the medical profession and other health-related facilities and organizations.

• Plan, assist, develops, organize, implement, evaluate, and direct the social service programs of the facility.

• Understand and communicate surge plan information to medical records.

• Monitor and evaluate the utilization of psychotropic medications and related behaviors and update care plan when necessary, including scheduling short-term care plan reviews, gradual dose reductions, and referrals to MSW, Psychologist, and Psychiatrist.

• Review complaints and grievances along with lost property reports made by the resident/family and make a written report to the Administrator indicating what action was taken to resolve the complaint or grievance.

• Participate in resident/family council meetings as requested and provide support services to such council.
