This is a Neighborhood Health Center position

The social worker will advocate for participants and families that are referred from Nevada Health Solutions and the Culinary Health Centers to assist in resolving healthcare/societal related issues in an emergent situation. The social worker will assist participants and families by helping them cope with and solve social issues in their everyday lives. The social worker will help participants and families who face a disability, life-threatening disease, social barriers, such as inadequate housing, unemployment, or substance abuse. Additionally, they may conduct research and advocate for improved services.

ESSENTIAL JOB FUNCTIONS AND DUTIES

• Provide psychosocial support to individuals and families so they can cope with chronic, acute, or terminal illnesses.
• Advise family caregivers, counsel patients, and help plan for patients' needs after discharge from hospitals.
• Act as liaison and referral source to the on-site and in-house case management team to help manage the social components of complex patient needs
• Make referrals to appropriate agencies i.e. child protective services, WIC, Medicaid office, etc.
• Work with the Union, TPA, and vendors (ex; Harmony) to identify and facilitate non-clinical needs of the participants and families.
• Participate in health fairs, enrollment events, etc.
• Develop and maintain a community resource manual
• Work with the participants, physicians, and TPA to facilitate the completion of disability and loss of time paperwork.
• Develop programs to combat social problems, and/or improve community health and counseling services.
• Driving on Fund business; Daily driving within the Las Vegas Metropolitan area based upon assignment to hospitals and/or facilities and the culinary health centers
• Participate in the weekend on call rotation

ESSENTIAL QUALIFICATIONS

• 3 ~ 5 years of direct experience in healthcare or managed care environment
• Working knowledge and experience in a Union operations and/or Taft-Hartley environment
• Solid understanding of medical benefit plans
• Familiarity with common medical conditions, treatments and diagnostic procedures (e.g., diabetes, hypertension, cancer screening, mammography, vaccination, prenatal care, etc.)
• Possess a strong desire to help improve people's lives.
• Bachelor's degree in Social Work required (preferred Master's degree)
• Required:Licensed social worker in the State of Nevada
• Required: Valid, unrestricted state driver's license
• Required: Personal vehicle registration and insurance

Salary range for this position: Salary $72,600 - $90,800. Actual base salary may vary based upon, but not limited to: relevant experience, qualifications, expertise, certifications, licenses, education or equivalent work experience, time in role, peer and market data, prior performance, business sector, and geographic location.

Work Schedule (may vary to meet business needs): Monday~Friday, 8 - 4pm, 7.5 hours per day (37.5 hours per week) full time onsite in our health center locations.
