PRN Social Worker at Surrey Place

Description

Evaluate and assess applicants/residents and/or family members for medical, emotional, social, financial and cultural background for placement and discharge in accordance with the policies of the Social Services Department. Performs duties and responsibilities is a manner consistent with our mission and values.

Principle Accountabilities:

1.Participates in the recruitment, selection, and placement of residents. 5%

2.Interviews applicants and/or families and obtains pertinent information regarding resident's medical, emotional, social, financial and cultural background. 14%

3.Evaluates information obtained. This includes an assessment of the resident's medical and psycho-social data such as behavior patterns, use of defense mechanisms and family relationships; assessment of resident's and family's strengths and weaknesses; analyzes factors which exert an adverse effect upon health problems, treatment, recovery, and adjustment to Surrey Place. 5%

4.Prepares psycho-social assessments no later than 7 days after admission for Medicare Part A and Medicaid residents. Private Pay within 14 days of admission. 5%

5.Documents in the Medical Chart, resident's plan of care, This includes social and emotional needs of resident as they relate to illness; response to treatment, and adjustment of the care provided; setting short and long range goals every 6 months. 5%

6.In compliance with the Patient Self Determination Act of 1990, all residents are advised of their right to complete advance directives upon admission. Social Service Department will also update the advance directive with the resident annually or on request. 3%

7.Completes Social Service portion of Medicare MDS forms and collects all other department MDS forms to comply with Medicare guidelines. 8%

8.Ensures that all new admissions and/or their families are aware of the resident's rights and receive a copy. 5%

9.Writes summaries and progress notes in resident's chart promptly. 10%



  1. Explains to resident/or family member, upon admission, the policy for CPR and elicits the resident and/or family member's consent on CPR form. All DNR's to be sent to resident's physician for signature. 1%

  2. Prepares Discharge Plan. 5%

  3. Collaborates with local agencies such as social security, state Division of Family Services and other agencies in finding appropriate resources/services for residents. 5%

  4. Collects and distributes information on other community health and welfare resources. 1%

  5. Assists with discharge planning to enable the resident and/or family to follow recommendations of physician. Gathers appropriate information to expedite discharge process. 10%

  6. Provides preventive, educational, evaluative and treatment services meeting psychosocial needs of families and residents to help them improve, restore and maintain their level of health and well being. 5%

  7. Engages in therapeutic interviews with residents, groups, families for the purpose of helping them to attain the maximum level of care and social functioning through mutually agreed upon goals 5%

  8. Initiates educational and therapeutic groups where needs may best be met by this treatment modality. 1%

  9. Participates with other members of the clinical team in the development and formulation of the resident care plan. 1%

  10. Participates in weekly Resident care Plan Conferences for the purpose of assisting, planning and treating residents and/or families. 3%

  11. Participates in Inservice and Educational Programs for the purpose of interpreting the roll and function of the social worker on the health case team. 1%

  12. Functions on appointed committees. 1%

  13. Participation in professional organizations. 1%

  14. Performs other related duties. 1%


Education

Masters Degree in Social Work required

Experience

Requirements for job: Masters Degree in Social Work, with experience in a hospital or skilled nursing facility preferred.

Employee must be able to communicate orally, in writing and electronically. Employee must be able to listen and to understand and explain complex situations. Must possess group work skills and collaboration skills.

The assessment, care and treatment provided by the Social Worker will be consistent with the specific age related needs of the patient. The Social Worker is competent to care for patient age groups including the neonates/infants, children, children, adolescents, adults and geriatric adult patient.

Working Conditions

Employee is required to sit frequently and walk occasionally.

Employee must lift/carry a light (1-20 lbs.) load occasionally.

Employee may be required to lift over 20lbs. Employee is occasionally required to bend, squat, crawl, kneel, climb, lift waist to overhead. Employee is frequently required to reach forward and reach overhead.

For repetitive action, employee is required to use both hands for simple grasping and pushing/pulling. Employee is occasionally exposed to electrical hazards and CRT terminal work. (Occasionally: 1-33%, Frequently: 34-66%, Continuously: 67-100%) The most significant of duties are included but this does not exclude occasional work assignments not mentioned. Job Relationships:

Works in active partnership with the MD's, NP's and RN's and other disciplines to develop and implement a well-coordinated personalized plan to address patient needs across the continuum. The social worker assesses plans and facilitates options and services to meet an individual's unique and complex needs. This is accomplished by addressing both the individual patient's medical and psychosocial needs, as well as the resources available. There is a continuing commitment to maximizing the quality of life for each patient and enhancing community health. The Social Worker performs duties and responsibilities in a manner consistent with the hospital mission, values and NASW Code of Ethics. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patients served. Principle Accountabilities: % of Time Spent 1. Utilizes department distress rating scale screening process, information from patient care plans, and built in EMR triggers to screen for/identify appropriate social work referrals. 10% 2. Completes psychosocial assessment consistent with department guidelines. Able to define patient continuum of care needs. Requires knowledge of human development and behavior, of social economic and cultural institutions and their interactions as part of the patient/family system. 10% 3. Responsible for coordination and implementation of continuum of care needs and insuring they are identified and addressed. Identifies appropriate education needs of patients utilizing in-house and community resources. 20% 4. Provides targeted therapeutic interventions for identified problems. Focus on the adaptive capacities of patients/families/caregivers with the goal of preventing maladaptive behavior and enhancing recovery. Appropriately assesses and intervenes in situations of abuse, neglect, domestic violence and sexual assault. 20% 5.
