SUMMARY

The Social Worker assists patients and families to resolve all aspects which may prohibit or limit the patient from receiving maximum benefit from medical care. Evaluates social-emotional functioning of patient and families, providing counseling when necessary and facilitates the discharge plan. Works cooperatively with other hospital departments, physicians and outside agencies. Provides assistance to neonate, pediatric, adult and geriatric patients.

KNOWLEDGE

1.Work requires a Bachelor's degree in Social Work with licensure from the State of Ohio as a Licensed Social Worker.

2.A relatively high level of analytical ability is required to assess patient needs, develop and manage discharge plan and to effectively utilize community and governmental resources.

3.A significant level of interpersonal skills is required to counsel patients and families concerning sensitive emotional and social issues.

4.Six to twelve months previous experience is required to acquire diagnostic interviewing and counseling skills.

WORKING CONDITIONS

1.Work is generally performed in an office or normal patient care environment where there is some exposures to communicable diseases.

2.On call responsibilities as assigned.

RESPONSIBILITIES

Travel Involved. Work with clients including home visits. Hours vary but generally 8a-4:30p Monday through Friday.

1.Conducts interviews with patients and families to obtain social work assessment and evaluation relevant to medical problems and discharge plan. Collaborates with entire health care team to ensure maximum benefits of medical care.

2.Completes social history and documents, when necessary on patients.

3.Evaluates data and refers patients to appropriate community resources, and if necessary, facilitates placement. Coordinates the exchange of medical and social information necessary to ensure continuity of care.

4.Provides counseling to patients, as needed, to assist them in decision making and acceptance of illness and related problem, e.g. Advance Directives, adjusting to disfiguring illness, permanent disabilities.

5.Maintains ongoing and develops new contacts with outside agencies in order to be used as a referral source.

6.Coordinates financial referrals and counsels patients and their families informing them of various assistance programs and community resources.

7.Participates as a team member in multi-disciplinary conferences.

8.Maintains JCAHO and Department standards. Prepares and submits monthly statistics as directed.

9.Provides service to all age groups. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patient served on assigned unit.

Demonstrates knowledge of the principles of growth and development over the life span by completing continuing education program.

Interprets information need to identify each patient's requirements relative to his/her age-specific need:

a. Neonate

b. Pediatric

c. Adolescent

d. Adult

e. Geriatric

Documents records for QA/QC purposes, checking functions and/or temperature of equipment and ordering of supplies. KNOWLEDGE Hand-eye coordination sufficient to perform technical duties described. Color discrimination sufficient to verify diagnostic quality of staining processes. Analytical skills sufficient to gather, organize, evaluate and integrate information for the purpose of determining a course of action. Knowledge of histochemical reagents, solutions and equipment operation and maintenance. Work is generally performed in a laboratory environment in which there is risk of exposure to unpleasant and/or potentially hazardous chemicals and biohazards. Physical exposure to cold chamber used in frozen section procedure and sharp blades used in cryostat and microtome. Work may be stressful due to production and high degree of accuracy demands. REQUIRED Associate's Degree or equivalent from two year college or technical school; or two years of clinical training in an accredited histology lab. * Knowledge of biology, chemistry and math acquired through one year's related course work or a certified histotechnician program * Certified Histotechnician is preferred Principal Duties and Responsibilities Ensures patient/specimen identification at all times. Receives and prepares tissue specimens for gross examination. Assists pathologist in gross examination and photography procedures. Performs a variety of clerical duties such as billing, accessioning, labeling and data entry, for the purpose of preparing specimens for further processing. Processes, embeds, cuts and mounts tissue samples to assure quality slides for microscopic evaluation. Prepares unfixed tissues for rapid diagnosis through freezing chamber (cryostat); stains and coverslips this tissue. Prepares chemical reagents and solutions for staining procedures. Performs routine histochemical and immune histochemical staining of tissues for interpretation by a pathologist. Properly identifies microscopic tissue structures and staining characteristics during and after staining procedures in order to guarantee properly stained slides. Checks function and/or temperature and maintains/cleans equipment according to procedure manual to assure properly functioning equipment. Files and maintains slide and tissue block archives in order to assure immediate retrieval of slides or blocks when necessary. Prepares cytology specimens. Refers patient identification discrepancies, unusual situations or specimens to Pathologist or Supervisor for clarification. Utilizes continuing education to keep current in histological procedures and technology. Demonstrates behaviors consistent with stated core values of Mercy Health Partners.
