Shriners Children's, the premier pediatric burn, orthopaedic, spinal cord injury, cleft lip and palate, and pediatric subspecialties medical center, has an opportunity for a Social Worker to join our team.
Job Overview
The Social Worker is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.
Responsibilities
Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.
Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
Collaborates by working with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve, and share accountability for optimal outcomes.
Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.
Qualifications
Education
Masters in Social Work required
Required experience
Certification
Social Work license in state of practice
Current Case Management Certification (CMC, ACM or ANCC) is preferred, or the ability to attain certification within 12 months of hire or 12 months of eligibility
Additional knowledge and skills:
Bilingual in English/Spanish preferred
Demonstrated competency in Microsoft Office Suite required
