Position Overview:
Provides clinical Social Work Services necessary to identify and assist in meeting the psychosocial needs of all residents, family members and others as appropriate or necessary in coping with the effects of illness, treatment, rehab stay or transition to placement in the Skilled Nursing Facility.
What you'll do:
Communicates with, and coordinates efforts between physicians, nurses, and other personnel regarding resident care plan and/or discharge plan as appropriate
Provides education and support to residents and/or families regarding topics of dementia progression, advanced care planning, end-of-life decisions, etc.
Documents timely and accurately the medical record the resident psycho-social assessments and all services provided to the patient and family in accordance with state and federal regulations.
Attends and actively participates in weekly Comprehensive Care Planning and Discharge Planning meetings.
Maintains relationships with other health care facilities, community agencies, and other appropriate organizations as required in order to build relationships and assist residents with access to community programs and services.
Assists the Manager in Departmental functions, including coverage in the absence of the Manager, Resident Council activities, religious programming, special event planning, and other duties as assigned.
Participates in Quality Assurance efforts throughout the facility.
