Responsibilities

The Clinical Social Worker is responsible for:

  • Providing social work services to all customer groups affected by illness/disability, often including lifestyle adjustment difficulties

  • Understandsing utilization criteria and case management principles in a healthcare setting, as well as relevant community resources, insurance, regulatory, and legal system knowledge

  • The Clinical Social Worker reports and is accountable to the Director of Case Management and Social Work

  • Conducting Social Work assessment/counseling/education Services

  • Providing discharge planning services

  • Collaborating with inter-disciplinary team

  • Professional behavior and development

  • Documenting professional activity

  • Providing information/referral services for community resources

  • Demonstrating a commitment to patient safety

  • Acting as a Corporate citizen

Qualifications

The Clinical Social Worker must:

  • Have a minimum of a Master's in Social Work

  • Have at least 3 to 5 years experience of social work in an acute care, IRF, or LTACH setting, medical crisis intervention, and knowledge of community resources or related experience is preferred

  • Be state-licensed as an LCSW in the state in which facility resides

Martin L King Blvd. Las Vegas, NV 89106 P: 702-382-5580 Full Time Position Summary: The Licensed Social Worker will assist in the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards as well as our established policies and procedures to ensure that the medically related emotional and social needs of patients and/or residents are met and maintained on an individual basis. They will develop and manage the initial contact as necessary with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures. The Licensed Social Worker will also develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients/residents. Finally, they will keep abreast of current federal and state regulations as well as professional standards.
