The Clinical Social Worker is responsible for:
Providing social work services to all customer groups affected by illness/disability, often including lifestyle adjustment difficulties
Understandsing utilization criteria and case management principles in a healthcare setting, as well as relevant community resources, insurance, regulatory, and legal system knowledge
The Clinical Social Worker reports and is accountable to the Director of Case Management and Social Work
Conducting Social Work assessment/counseling/education Services
Providing discharge planning services
Collaborating with inter-disciplinary team
Professional behavior and development
Documenting professional activity
Providing information/referral services for community resources
Demonstrating a commitment to patient safety
Acting as a Corporate citizen
The Clinical Social Worker must:
Have a minimum of a Master's in Social Work
Have at least 3 to 5 years experience of social work in an acute care, IRF, or LTACH setting, medical crisis intervention, and knowledge of community resources or related experience is preferred
Be state-licensed as an LCSW in the state in which facility resides
