Summary To respond to each resident social needs from the time of admission and throughout the resident#s stay, collaborating with other staff and professionals both in and outside the facility in developing and implementing coordinated individualized plan of care and supportive services. To respond to each resident social needs from the time of admission and throughout the resident#s stay, collaborating with other staff and professionals both in and outside the facility in developing and implementing coordinated individualized plan of care and supportive services. Assists with Advance Directives, and future plans including discharge plans. Respond to the needs of the resident and family by developing a trusting relationship.

Assess and document the psychosocial needs of the resident, developing goals and working towards a resolution of the goals with the resident and/or family. Responds to the needs of the resident and family.

Provide emotional support in a caring and sensitive manner.

Develops a complete social history for each resident to establish information for planning. Provides, refers or implement other options for counseling services to residents and families in order to ensure adjustment to placement, illness or disability; coping with issues related to aging and loss. Schedules the use of community referrals when appropriate.

Participate in the care planning process including completing the MDS, writing individualized goals, arranging for family participation in the care plan meetings, and jointly facilitating the care plan meeting with the unit manger. Assess the resident#s ability to be discharged through collaboration with the resident, family, and interdisciplinary team. At time of discharge, arrange for equipment and community services to ensure a seamless transition.

Advocate for the resident, effectively communicate Resident Rights, communicate important information related to the resident#s orientation, and be a role model to other staff related to resident rights. Demonstrate knowledge of insurance and financial programs, including Medicare and Medicaid, that would affect the resident#s status, in order to assist residents and determine appropriateness for admission. Complete all computer documentation including admission assessments, quarterly progress notes, care plan goals, discharge plans and other appropriate documentation that represents the residents stay.

Collaborates with care management team, admissions, department manager and business office to ensure a high satisfaction level during the admission process. Assist resident/family with end of life decision making and Provide educational materials, answer questions, and assist with making hospice referral. Provide education and assist resident/family in completing POLST, Living Wills, and Power of Attorney.

Identifies and suggests innovative approaches and participates in implementing performance improvement opportunities. Education and/or Experience Bachelor#s degree from an accredited college or university or four years of relevant experience in a long-term care community, or an equivalent combination of education and experience. Degree in Social Work, Gerontology or other related field preferred.

Computer Skills To perform this job successfully, an individual should have knowledge of Internet software; Spreadsheet software and Word Processing software. Certificates, Licenses, Registrations Social Work Licensure in PA preferred #
