The Social Worker provides quality psychotherapeutic and discharge planning services to all patient populations and their families; provides assessment, group treatment, family work, and discharge planning; serves as a member of interdisciplinary team supporting the organization's treatment program and philosophy.

In addition to its main location in Philadelphia, the hospital has a growing network of primary and specialty care locations throughout the Philadelphia suburbs and New Jersey so your child's health conditions can be treated close to home. Responsibilities This position will encompass three areas in General Pediatrics (Center for the Urban Child; GROW Clinic and Collaborative Primary Care). This position will require the incumbent to "float" between each area. The majority of time will be in the Center for the Urban Child (CUC). The CUC is a large Pediatric Primary Care Practice Resident teaching practice. There are numerous programs within the practice (e.g. GROW Clinic, Collaborative Primary Care, Medical/Legal Partnership, Reach-out-and-read, etc.) The Social Worker will provides services to patients and families for ongoing care. As a member of an interdisciplinary team, the Social Worker assists families and children through advocacy, concrete services, counseling and support. He/she is a liaison to community agencies and referral sources. The Social Worker will also provide consultation and/or training to the professional community. He/she will also work in conjunction with behavioral health agencies in assessing/providing therapeutic interventions. This includes, but is not limited to, individual counseling and support groups. He she/is required to: * Facilitate interdisciplinary team meetings. * Community engagement. * Participate/initiate research opportunities. * Assist parents/guardians with appointment scheduling, referrals and follow up along with the members of the interdisciplinary team. * Participate in Practice Intake Coverage. * Collaborate with Social Workers, both inpatient and outpatient. * Supervise MSW Interns. * Program development/implementation (internal/external). * Participate in Provider and Resident Education. In the Grow clinic, the Social Worker serves children with feeding difficulties (e.g. Failure to thrive, psychosocial issues, etc.) He/she will also carry a small caseload of Parent Child Interactive Therapy (PCIT) patients. This position works closely with team psychologist (and students) in ensuring efficacy of practice and competency of those providing this treatment. Collaborative Primary Care operates as a Medical Home for patients with high-risk social needs within the Center for the Urban Child. The social worker provides therapeutic intervention and supportive services/resources to patients and families. This role would also include screening of patients/families, which may be identified by the larger Social Work and/or Medical team, as being at risk based on their medical and social needs. These needs include, but are not limited to, social determinants for issues such as, mental health, poverty, food insecurity, intimate partner violence, medical and physical neglect/abuse (Child Welfare agencies). Referrals would be made to appropriate community/social agencies. Qualifications Skills & Abilities: Knowledge of developmental stages of children and adults. This position interacts with patients from all ages from Neonates to Adults. Knowledge of basic computer skills is an asset. Training & Education: Masters Degree from an accredited school of Social Work. This position interacts with patients from all ages from Neonates to Adults. Experience: Minimum 5 years graduated experience under MSW supervision preferred. Supervisory Responsibility: Potential for supervising Masters prepared Social Work interns.
