PTS, Inc. is seeking a Social Worker to provide school-based services for a local school district in New CastleCounty, DE:

  • Full-time or part-time available; hours fall within a normal school day and will vary depending on primary and secondary caseloads.
  • This position will be for the 22-23 school year.

Responsibilities:

Provide comprehensive school social work services to students, parents, and school staff, addressing barriers that limit a student from receiving full benefit from their educational experience. Respond to referrals from school administration, parents, teachers, Special Education team, and others by providing direct services and by assisting families in accessing appropriate community resources.

  • Serve on students multi-disciplinary teams at assigned schools
  • Review student records to identify previous barriers and/or interventions.
  • Conduct parent interviews to acquire socio-developmental information as needed for special education referrals.
  • Analyze information gained through record reviews and interviews to determine environmental impacts and appropriate resource needs and report data per district preferred method
  • Attend and participate in interdisciplinary meetings to promote collaborative approach to treatment
  • Interview students and parents to discuss issues related to non-attendance and develop a plan of action.
  • Assist the school in developing, maintaining, and or enforcing policies related to excessive absences and truancy; attend student support team meetings
  • Develop and implement professional development training for educational staff and parents, as requested.
  • Provide individual and/or group counseling in response to school-wide crises.
  • Respond to referrals requesting verification of residence.
  • Participate in professional development activities aimed at current trends and best practices for the provision of comprehensive school social work services.
  • Conduct home visits as a method to access the family and conduct interviews in response to school referrals.
  • Maintain an ongoing liaison with community agencies and other resources to meet student needs; refers parents and students to agencies when appropriate.
  • Perform related work as required.

Qualifications:

  • Masters degree in Social Work
  • Valid state license in Social Work

Since class specifications are descriptive and not restrictive, incumbents can complete job duties of similar kind not specifically listed here.</p><ul><li><p>Interviews client, family and/or professional sources to determine clients needs for a variety of social services which may include employment, job search and basic skills remediation.</p></li><li><p>Develops a comprehensive cost effective service plan utilizing in-house sources, referrals, contracted providers and/or institutions.</p></li><li><p>Conducts on-going monitoring and assessment of service delivery for optimum quality and efficiency, and authorizes and recommends changes and adjustments as needed.</p></li><li><p>Maintains comprehensive case records, summaries, statistical and narrative reports and completes required forms.</p></li><li><p>Establishes effective working relationship with clients, family, community and professional sources.</p></li><li><p>Provides specialized casework and counseling services in such areas as protective, adoption and options counseling.</p></li><li><p>Provides direct casework service through individual/group conferences, counseling and/or site visits.</p></li><li><p>Supplies information and responses to inquiries regarding services for clients, family, community and government agencies.</p></li><li><p>May provide supportive casework to clients prior to, during or after hospitalization based upon physicians diagnosis and recommended treatment.</p></li></ul><p>Job Requirements</p><p>JOB REQUIREMENTS for Senior Social Worker/Case Manager</p><p>Applicants must have education, training and/or experience demonstrating competence in each of the following areas:</p><ul><li><p>Six months experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individuals human service needs.</p></li><li><p>Six months experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.</p></li><li><p>Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.</p></li><li><p>Six months experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.</p></li><li><p>Six months experience in narrative report writing.</p></li></ul><p>Additional Posting Information</p><p>Due to a new requirement from the Centers for Medicare &amp; Medicaid Services (CMS) the following mandate is effective January 27, 2022: As a condition of employment, employees providing health care services and/or support services in covered facilities must show proof that they have received the dose of a single J&amp;J dose or the 1st dose of Moderna/Pfizer two-dose vaccine effective date January 27, 2022, and proof of the single J&amp;J dose or the 2nd dose of Moderna/Pfizer two-dose vaccine by February. 28, 2022.
