We're looking for someone to join our team as a Social Worker (PD) who:

Provides services to patients and their families to deal with needs incidental to their hospitalization. Provides counseling, support and assistance in making transition plans. Provides direction for routine aspects of non-medical problems of patients and their families. Assists with the planning and post-discharge of patients with outside agencies, nursing staff, and relatives.

Essential Functions:

  • Collaborates and facilitates patient discharge plans for those requiring post Hospice or post inpatient care.

  • Assists patient/family/caregiver in determining appropriate level of Hospice care. Refers to outpatient resources as appropriate.

  • Initiates a plan of care and appropriate patient teaching. Reassesses plan of care according to the physical and emotional needs of patient.

  • Develops Plan of Care (POC) based on patient assessment including, psychosocial factors that would impede the POC, clinical knowledge of appropriate theories of stages of human development, the impact of terminal illness, mental status and family dynamics.

  • Performs other job-related duties as assigned.

You'll be successful with the following qualifications:

Education:

  • Bachelor's Degree or equivalent (some certifications will require Bachelor's): Required

  • Master's Degree in social work or equivalent combination of education/experience: Preferred

Work Experience:

  • Experience in an acute or long term medical setting: Preferred

Required Licenses/Certifications:

  • Licensed Clinical Social Worker (LCSW): Preferred

As such, it is a one-stop destination for our faculty, staff, and their dependents, providing confidential counseling, coaching, consultations, programs, and referrals across a wide range of personal and workplace concerns. The CWFL has an opportunity for a part-time (1 day/week, with anticipation of an increase to 2 days/week) Social Worker. The Social Worker will provide a full range of clinical services to include assessment of individual and/or departmental needs, short term treatment, referral, crisis intervention and psycho-educational training. Contributes to design of program content and policies addressing family, personal and work related issues. Develops and conducts training sessions for faculty and staff to increase awareness and skills in dealing with chemical dependence, stress and other issues affecting employee morale and performance. Evaluates programs on an ongoing basis, including bilingual and bi-cultural activities. Provides supervision to graduate students. Additional accountabilities include: * Conduct individual, couple, and family assessment and diagnostic sessions. * Reviews and evaluates client progress and establishes follow-up contact. * Consults with department administrators regarding problems presented by staff and/or faculty related to psychosocial factors. * Manages the delivery of services to targeted program participants or beneficiaries. Minimum Education: Master's degree Minimum Experience: 2 years, Combined education/experience as substitute for minimum experience Minimum Field of Expertise: Social work.
