AuthoraCare Collective is currently seeking a Social Worker Palliative Care for our Greensboro service areas. This is a full-time position Monday

  • Friday 8:00am

  • 5:00pm.

Job Summary:

Responsible for providing comprehensive assessment of the social, emotional, and practical needs of the Palliative Care and other program's patients and family members. Provides advanced counseling, education, and referrals to address patient/family issues of grief and loss, adjustment, care needs and end of life planning. Functions as a member of an interdisciplinary team of care in collaboration with external partners. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

Education/Qualifications:

  • BSW or MSW

  • One year of experience in social work in a mental health or medical setting is required.

  • Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

  • Ability to concentrate with large volumes of work, handle pressure of deadlines, good judgment, ability to follow procedures, ability to work independently and with frequent interruptions.

  • Provide effective counseling to patients and their families, communicate effectively with persons having highly diverse backgrounds and educational levels.

  • Work effectively as a member of the interdisciplinary team.

  • Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
