This position is assigned to the Ambulatory Care areas, providing Medical social work services to Veterans/families and caregivers assigned to the designated PACT providers. The Social Worker provides a full range of social work services and treatment of complex psychosocial problems resulting from chronic and acute illness, senescence or socioeconomic and family concerns.
This includes assessment and intervention tailored to the unique needs of the chronically ill and aged patient population. The Social Worker has knowledge regarding the symptoms and treatment of Depression, Military Sexual Trauma, PTSD, Substance Abuse and Suicidality and screens for these and other mental health issues to ensure appropriate referrals prompt treatment and needed follow-up. Screening and intervention regarding advance planning documents is also executed.
Conducts psychosocial assessments and provides psychosocial interventions/treatment.
Identifies the appropriate resources to fit the needs of the Veteran.
Works with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling skills.
Establishes and maintains effective working relationships with clients, staff, and representatives of community agencies.
Utilizes computer software applications for drafting documents, data management, and tracking.
Provides consultation services to other staff about the needs of patients and impact of psychosocial problems on health care and compliance with treatment.
Performs other related duties as assigned.

Minimum of one year of post-MSW degree experience in the field of health care social work.
