The Complex Case Management Program is consistent with the case management industry guidelines: assessment, care planning, communication and coordination, education, empowering and advocacy. The goal of this program is to improve quality outcomes and optimize service utilization and cost.

  • The assigned Social Worker (SW) will operate within the company's Clinical Care Delivery department providing onsite and telephonic care management services to individuals identified as "high risk members"

  • Serve as the primary point-of-contact for a selected group of members and their families

  • Will travel between the office, member's homes and acute and subacute facilities

  • Will participate in twice weekly meetings with Medical Director to review member's progress, update plan of care as needed

Primary Responsibilities

  • Obtains consent from the member/family for participation in Complex Case Management

  • Develops an outcome-based plan of care, based on the member's input and assessed member clinical and social needs with anticipated targeted dates for accomplishment

  • Initiates onsite hospital visits/rounds as needed to assess patient progress and meet with appropriate members of the patient care team

  • Conduct initial in-home or facility visit to engage and educate the member (and their family/caregivers) to the Complex Case Management program

  • Routinely conduct telephonic, in-home, and/or in-facility visits to monitor the member's adherence to their established care plans

  • Continuously coordinates, monitors, tracks and evaluates all care and services rendered to ensure that quality care is being delivered in the most appropriate setting

  • Routinely discuss members' care plans with the Medical Director to make adjustments as necessary

  • Identifies and addresses psychosocial needs of the members and family and facilitates consultations with Social Worker

  • Identifies and addresses pharmacological needs of the members and facilitates consultations with the pharmacy department, as necessary

  • Responsible for the coordination of post-discharge clinic appointments, PCP and specialist visits

  • Be available (during work hours) to take incoming phone calls from assigned members

  • Maintain a comprehensive list of available wellness and other community programs available to members and facilitate referral of members to resources

Requirements

  • Excellent communication skills to effectively interact with members, families, and external healthcare providers

  • Ability to work effectively both on an individual basis and as part of a multidisciplinary team

  • Strong computer skills to maintain accurate and complete documentation in our Case Management record systems

  • Strong working knowledge of Milliman Care Guidelines

  • Highly organized and self-motivated to effectively manage multiple members

  • Adherence to all member privacy and confidentiality standards of practice (i.e. HIPAA)

Must-have qualifications and expertise include:

  • Masters in Social Work (MSW) required from a graduate school approved by the Council on Social Work Education

  • LMSW/LSW Required

  • At least 2 years of medical social work experience

  • Familiar with medical case management and healthcare

Facility Profile: Located on a lush tropical campus, our flagship hospital, 1,368-bed AdventHealth Orlando, serves as the major tertiary facility for much of the Southeast, the Caribbean and South America. AdventHealth Orlando houses one of the largest Emergency Departments and largest cardiac catheterization labs in the country. We are already one of the busiest hospitals in the nation, providing service excellence to more than 32,000 inpatients and 125,000 outpatients each year. Department Profile: The Care Management Department is located in every campus throughout the AdventHealth system. Our objective is to create a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for services to meet an individuals and family's comprehensive needs. Care Management is an integrated department with physicians, clinical/nursing teams, ancillary departments and the revenue cycle teams. Care management meets the needs of our patients, families and significant others throughout the care continuum including the post-discharge needs. Work Hours/Shifts: Monday – Friday; 8am – 5pm Job Summary: Under the general supervision of the Director and/or Manager, in collaboration with the RN Case Manager, works with the patient/family, physicians, interdisciplinary team and external agencies. The Social Worker-MSW ensures the patient's progress along the continuum in an efficient and cost–effective manner. Utilizes expertise in patient/family assessment incorporating physical, psycho-social, environmental, spiritual and financial factors to create a sound discharge plan to ensure proper patient placement across the healthcare continuum and to maximize capacity. Utilizes psychosocial assessment skills and crisis intervention techniques to expedite and support healthcare team's work with patient/ family. The Social Worker-MSW, in collaboration with the RN Case Manager, is responsible for identifying and initiating the most efficient discharge plan to expedite the patient transition to the next appropriate level of care and to reduce length of stay. This position is the organization's expert in assessing and managing the patient's psycho-social needs. The Social Worker-MSW educates the health care team and physicians about psycho-social issues and identified patient/family problems as well as recommends strategies to address these issues. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Documented assessments confirm an understanding of the patient's functional status and psycho-social needs. * Collaborates with the RN Case Manager to address barriers that may impact or delay the patient's discharge plan. * Prepares for and participates in multi-disciplinary rounds on assigned nursing unit for the purpose of updating the team on the discharge plan/needs and to identify potential social work referrals. Appropriately prioritizes cases according to assessed needs. * Performs discharge planning/post-acute care coordination to include: * Acute rehabilitation placements * New nursing home or skilled nursing placement * Psychiatric or substance abuse placements * Cases involving active psychiatric conditions * New dialysis * Acute care transfers * Abuse cases * Hospice referrals and placements * Legal issues: adoptions and guardianship cases in collaboration with legal counsel * Specialty program participation (Transplants) * Assistance with Advance Directives, as necessary * Community resource needs * Financial issues/funding for post-acute needs/identifying possible MA eligibility cases with appropriate referrals * Provides patient/family education, adjustment-to-illness counseling, grief counseling and crisis intervention. Provides education to patients/families/caregivers regarding resource options and coping with diagnosis, treatment and prognosis. * Works in collaboration with hospital and community agencies to obtain needed services and resources for patients/families/caregivers. * Adapts quickly to changes in patient status and updates the assessment and discharge plan as the patient's situation warrants.
