The Complex Case Management Program is consistent with the case management industry guidelines: assessment, care planning, communication and coordination, education, empowering and advocacy. The goal of this program is to improve quality outcomes and optimize service utilization and cost.

  • The assigned Social Worker (SW) will operate within the company's Clinical Care Delivery department providing onsite and telephonic care management services to individuals identified as "high risk members"

  • Serve as the primary point-of-contact for a selected group of members and their families

  • Will travel between the office, member's homes and acute and subacute facilities

  • Will participate in twice weekly meetings with Medical Director to review member's progress, update plan of care as needed

Primary Responsibilities

  • Obtains consent from the member/family for participation in Complex Case Management

  • Develops an outcome-based plan of care, based on the member's input and assessed member clinical and social needs with anticipated targeted dates for accomplishment

  • Initiates onsite hospital visits/rounds as needed to assess patient progress and meet with appropriate members of the patient care team

  • Conduct initial in-home or facility visit to engage and educate the member (and their family/caregivers) to the Complex Case Management program

  • Routinely conduct telephonic, in-home, and/or in-facility visits to monitor the member's adherence to their established care plans

  • Continuously coordinates, monitors, tracks and evaluates all care and services rendered to ensure that quality care is being delivered in the most appropriate setting

  • Routinely discuss members' care plans with the Medical Director to make adjustments as necessary

  • Identifies and addresses psychosocial needs of the members and family and facilitates consultations with Social Worker

  • Identifies and addresses pharmacological needs of the members and facilitates consultations with the pharmacy department, as necessary

  • Responsible for the coordination of post-discharge clinic appointments, PCP and specialist visits

  • Be available (during work hours) to take incoming phone calls from assigned members

  • Maintain a comprehensive list of available wellness and other community programs available to members and facilitate referral of members to resources

Requirements

  • Excellent communication skills to effectively interact with members, families, and external healthcare providers

  • Ability to work effectively both on an individual basis and as part of a multidisciplinary team

  • Strong computer skills to maintain accurate and complete documentation in our Case Management record systems

  • Strong working knowledge of Milliman Care Guidelines

  • Highly organized and self-motivated to effectively manage multiple members

  • Adherence to all member privacy and confidentiality standards of practice (i.e. HIPAA)

Must-have qualifications and expertise include:

  • Masters in Social Work (MSW) required from a graduate school approved by the Council on Social Work Education

  • LMSW/LSW Required

  • At least 2 years of medical social work experience

  • Familiar with medical case management and healthcare

Responsibilities * Performs patient and family assessment, along with the initial bereavement assessment within five (5) days of admission to hospice. Collaborates with the Interdisciplinary Team (IDT) to formulate the Plan of Care (POC). * Discusses the POC with the admitting RN on the day of admission for new patients. * When requested by the patient/family conducts regular social work visits and documents visit. * Evaluates insurance coverage on patients and assists patient/family with obtaining Medicaid or other insurance coverage when the patient does not have a payer source. * Participates in the IDT Meeting and updates the POC at least every 14 days. * Responsible for facilitating discharge and transfer of patients to other healthcare providers when hospice care is no longer requested or appropriate and completing appropriate paperwork. * Participates in the Performance Improvement Process as requested by the Administrator and/or the Patient Care Coordinator. * Identifies psychosocial problems and establishes goals that are pertinent, personalized, and measurable as evidenced by documentation. * Demonstrates knowledge of the Hospice Medicare and Medicaid Benefit and is able to convey knowledge to patients and families. * Conducts after death bereavement assessment. * Explains Advanced Directives to Patients/Families and communicates and documents patient decisions to the patient's attending physician. * May be asked to assist with in-services to vendors. * Assists patient/family with making funeral arrangements, if requested. * Knowledgeable of federal, state and local resources for patients, when appropriate, and assists patients/families in obtaining these resources. * Willing and able to accurately utilize any automated device required to document in the electronic medical record. * Communicates with physicians regarding changes in patient's condition and documents interventions. * Must be willing and available to make after hours visits (i.e. on weekday evenings and/or weekend) when needed for a patient's admission, death or funeral, etc. Qualifications Requirements: * Tennessee and Missouri = MSW preferred, minimum BSW required * Virginia = Social Work Licensure required; LMSW preferred, Min. LBSW required * South Caroline = Social Work Licensure Required; LMSW required * Georgia = Social Work Licensure required; LMSW preferred, Min. LBSW required * Minimum requirement is a Baccalaureate degree in Social Work - This is only available as long as Caris is able to arrange ongoing supervision and oversight by an MSW * One (1) year of social work experience in a health care setting.
