Work Days and shift:
Day shift, weekdays.
Social Worker (MSW) Will Be Responsible For:
Assist the physician and other team members in understanding the significant social and emotional factors related to the health problem.
Participates in developing a plan of care and participates in discharge planning.
Acts as a consultant to home health staff.
Complies with accepted professional standards and principles.
Shares responsibility with other professional disciplines for evaluating and treating all patients admitted to Home Health.
Documents service delivery in clinical notes and progress notes.
What You Will Need:
Master's Degree from a school of social work accredited by the Council on Social Work Education. Licensure required in West Virginia and eligible for licensure in the States of Ohio and Pennsylvania. Valid driver's license, proof of auto insurance, and willingness to drive in inclement weather required. Must be able to clear criminal background check. Knowledge of community resources and mental health systems required. Demonstrates ability to apply medical social work and home health social work practices and procedures, knowledge of Medicare and Medicaid regulations.
One year of social work experience in a health care setting required, home care experience preferred.
