Atlantic Health System is currently seeking a per diem Social Worker for Continuum Care Department at Morristown Medical Center.

Hours are 8am-5pm, must be avaliiable for 3 shifts per week, shifts are not guaranteed, must provide avaliability 3 months in advance. 2 weekday openings, 1 weekend opening with 3-4 shifts per month.


Essential Functions/Responsibilities of Position:

  • Responsibilities will include discharge planning for post-acute needs, insurance authorizations for sub-acute and rehab facilities, conduct psychosocial assessments, family meetings and acting as an integral part of reducing Length of Stay.

  • Supportive counseling, cirsis intervention, referrals for abuse and neglect ( adults and pediatrics), end of life counseling, supprotive diagnosis.


Minimum Education Required:

  • Master's in Social Work from an accredited school of social work

Minimum Experience Required:

  • NJ Social Work License LSW or LCSW required.

  • Previous hospital social work ecperience preferred.

  • Familiarity with Epic preferred.

This incredible opportunity is full time; a total of 37.5 hours a week. Must be familiar with resources and services available in Sussex and Warren Counties. Responsibilities Duties & Responsibilities: * REFERRALS & SERVICE IDENTIFICATION: Responds to all referrals in accordance with department policy time frames. Participates in patient case rounds, confers with physicians, care team, identifies and prioritizes psychosocial problem areas and pt/family needing social work intervention. Maintains and utilizes a current knowledge base of community agencies and facilities. Refers patients/families to appropriate programs and community resources in a proactive and timely manner. Provides appropriate follow-up with patient/family on their efforts to follow through with referrals. Facilitates referrals for complex continuing care facilities/services. * CLINICAL SKILLS Obtains relevant essential data to complete a Psychosocial Assessment, Multidisciplinary Plan of Care/Critical Care Pathways and Discharge Form. Identifies relevant information regarding the patient/family medical condition, emotional status, functional abilities, barriers to learning, primary language, English language proficiency, cultural and religious factors, functional ability, financial situation/resources, and support systems to determine and carry out social work interventions. Provides counseling and supportive services to patients/families with choices, options, resources, and services to meet continuing care needs. Facilitates pt/family conferences with relevant medical staff to resolve issues/problems related to treatment, recovery, end-of-life issues and continuing care plans. Effectively uses a variety of therapeutic techniques and modalities in patient/family interventions to achieve optimal outcomes. Works collaboratively with psychiatric * COLLABORATION Ongoing collaboration and communication with Care Manager/interdisciplinary team to update and reassess changing psychosocial needs, medical needs, and plan of care. Proactively communicates with the social work manager regarding situations that interfere with the implementation of the patient s care plan/discharge plan. Provides consultation and assistance to social work colleagues. Liaison and patient advocate with community agencies, health care services, third party payers, and the hospital system. * DOCUMENTATION Charts clear and concise psychosocial assessment, relevant progress notes, educational record and multidisciplinary plan of care in accordance with department policy. Completes timely statistics in accordance to department policy. Completes appropriate forms to facilitate discharge/referrals. * FINANCIAL Obtains necessary financial information to facilitate continuing medical needs and determine eligibility for services/programs. Facilitates the timely completion of applications and appeals if patient/family is unable to do so. * LEGAL Educates and assists family in proactively identifying, initiating, and/or completing legal options (Health Care Proxy, Guardianship, restraining order etc.) In a timely manner, intervenes & arranges legal assistance for the incapacitated patient who is without significant others and/or financial resources. * ASSESSMENT OF SUSPECTED ABUSE, NEGLECT, AND INTIMATE PARTNER VIOLENCE Conducts assessment with patient/family regarding suspected abuse/neglect. Confers with referral source and health care team and appropriate others to gather information. Routinely screens for Intimate Partner Violence. Discusses safety planning, options, provides education about the impact of intimate partner violence on children and refers appropriately. * PROFESSIONAL AND DEPARTMENTAL DEVELOPMENT Utilizes constructive insights, feedback & criticism for improved performance, professional awareness and development. Regularly attends and actively participates, in an appropriate manner, assigned intra-disciplinary and interdisciplinary forums, administrative meetings, staff meetings, supervisory sessions, and in-service training. Annually participates in a minimum of 3 educational programs on topics relevant to practice area. Participates in the department s Customer Service, Press Ganey, Performance Improvement activities and Balanced Scorecard Pillar development and monitoring related to care/service improvement. Maintains a current NJ social work license, LSW or higher, and is in good standing with the NJ Board of Social Work Examiners. * All other tasks or duties as assigned. * Physical Requirements: As defined by Occupational Safety/OMS Qualifications Required Education and/or Experience: * MSW from an accredited school required• 3-5 years Social Work experience required. • • Position will work with Care Coordination team to ensure patients/families are referred to appropriate programs and community resources and provide psychosocial and financial assistance and facilitate referrals for complex continuing care facilities/services.
