Job Summary:

Responsible for coordination of patient/client referral to community resources to facilitate discharge of HomeCare clients in a timely manner, and to facilitate problem solving of psychosocial issues impacting clients and the implications for provision of HomeCare/Hospice services. Develops a holistic, multidisciplinary plan of care utilizing the job functions within each of the five core components of case management: Coordination and Service Delivery; Physical and Psychological Factors; Benefit Systems and Cost Benefit Analysis; Case Management Concepts; and Community Resources. Interacts with RN case managers, physicians and interdisciplinary team to determine appropriate HomeCare/Hospice services for patients/clients based on regulatory guidelines for each line of service.

Job Functions:

1.Initiates and maintains communication with the attending physician, client and family, and interdisciplinary team members regarding assessment findings, changes in patient condition, and plan of care.

2.Reviews with RN case managers any changes in the plan of care to facilitate timely discharge to appropriate Lourdes Continuum service line.

  • Collaborates with other health professionals and paraprofessionals when necessary. Instructs and educates staff, client, client's family, and volunteers regarding psychosocial aspects of provision of care.

4.Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.

5.Supervises BSW social work staff in the provision of quality psychosocial home care. 6. Obtains data on an ongoing basis regarding psychological, social, and spiritual factors that may influence client health status.

  • Assesses cultural, spiritual, and educational factors and incorporates findings in provision of care.
  • Interacts with all customers in a pleasant, reassuring, and professional manner.

9.Responds to inquires/complaints and follows up appropriately.



  1. Meets regularly with interdisciplinary team to provide support and exchange feedback aimed toward enhancing professional growth.

  • Provides care in a safe and competent manner.
  • Provides a bridge of care linking the psychiatric client between the inpatient setting and the community that improves the quality of the client's life and maximizes the client's potential to live at home.

  1. *Assesses for home safety and provides teaching, information, referrals, and problem solving in order to optimize the client's functioning at home.

  2. *Values the client's right to self-determination; helps clients to establish goals and work through the decision-making processes involved in long-term care planning.

  3. *Provides individual and family counseling working through issues of caregiver noncompliance with medical regime; assessing families with newborns for bonding, postpartum depression, and other parenting issues; and assisting with appropriate community referrals.

  4. *Provide individual and family counseling related to dying process, palliative care and Hospice concept of care to help prepare patient and family for impending patient death. Assist in bereavement care and counseling as requested by interdisciplinary team.

  5. Participates and assists with orientation of RN case managers to social work concepts.

  6. Utilizes appropriate care standards to coordinate client care with documentation to support variances as they occur.

  7. Individualizes plan of care within care standards to effect maximal client outcomes.

  8. Works with RN case managers to implement discharge planning upon admission and coordinates discharge to appropriate Lourdes Continuum Service lines throughout care.

  • Consistently assesses client and client needs using appropriate integrated tools.

  1. Coordinates client conference to enhance full team discussion, consultation, and evaluation of plan of care.

  2. Participates in Utilization Review process to ensure accurate documentation of plan of care to assure accuracy of billing codes.

  3. Meets established productivity standards.

  4. Documents all care in reimbursable terminology to include coding, and timely submission of accurate certifications, recertification's, and verbal orders. 26. Maintains up-to-date client records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur. 27. Is knowledgeable of reimbursement principles and guidelines.

  5. Participates in agency and community programs to promote professional growth and understanding of HomeCare/Hospice.

  6. Has working knowledge of and utilizes community resources to ensure client needs are met.

  7. Links clients with informal community resources (church groups, neighborhood networks, volunteers) to maximize use of community resources. 31.


  • Promotes family and community involvement in meeting healthcare needs of clients.

  1. Seeks out and coordinates alternative care arrangement as client needs dictate.

Education Masters Social Worker degree required

Licensure / Certification Current Masters Social Worker license for Kentucky and Illinois. BLS certified. Certified Case Manager preferred.
