Principal Duties and Responsibilities * 1. Provides clinical consultations and psychosocial evaluations in a variety of inpatient and outpatient settings in a timely and comprehensive fashion in accordance with departmental standards. a.
Consults/assists with complicated medical diagnoses; complicated social issues; protective issues; legal/ethical concerns; financial concerns; and works with the health care team in facilitating discharge processes complicated by various factors. 2. Documents evaluations and ongoing work in a timely and comprehensive fashion in keeping with departmental standards. a. Initiates within 24 hours of referral for inpatients; same day when possible b.
Completes written evaluation within two sessions for outpatients c. Completes all appropriate forms within established time frames. 3. Provides ongoing individual, couples and family therapy as clinically indicated in the Life Resource Center and the Women's Health Services.
Provides clinical consultation in assigned outpatient areas as well (Multiple Sclerosis Clinic, Hematology/Oncology Clinic, Primary Care Clinic, Emergency Department/Walk-In Center, doctors' offices). Coordinates discharge planning, as needed, in the Emergency Department and Walk In Center. As the unit based social worker, provides consultative services to physicians and staff and facilitates case conferences as indicated. 4. Provides supportive therapy for patients and families relating to illness and/or hospitalization. 5.
Guides patients/families through hospital services, accessing needed health and community services, as indicated. 6. Utilizes supervision by bringing appropriate material for discussion and following up on recommendations made. 7. Participates in the training of social work students and trainees from other disciplines, in accordance with the hospital's commitment to train future health care professionals. 8.
Participates in the weekend oncall program, coming in as needed for ED and inpatient consultation/intervention. 9. Cross trains with other social work staff to enable adequate coverage. 10. Participates in appropriate hospital and community-based committees. 11. Conducts psycho-educational inservices in area(s) of expertise.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED 1. Master's degree in social work from an accredited school. 2.
One year of experience in a health/mental health setting preferred. 3. L.C.S.W. (or eligible), with plan for L.I.C.S.W. when eligible. 4. Excellent interpersonal and organizational skills.
Demonstrated ability to work collaboratively in interdisciplinary teams. 5. Ability to exercise sound clinical judgement in fast-paced and demanding settings.
WORKING CONDITIONS 1. Normal hospital and office environment(often shared offices). 2.
Occasional exposure to infectious disease. 3. Sitting approximately 65% of the time. 4. Walking required to different buildings. 5. Utilizing communication skills (listening and speaking) at least 85% of the time.
