Position Summary
The Social Worker, MSW is responsible for providing clinical social work services to patients, families and groups in all hospital areas specific to assignment. Provides professional services including but are not limited to bio-psychosocial assessment, brief treatment, crisis intervention, case consultation, patient education, discharge planning, and resource management. Additionally, this position is an integral part of the interdisciplinary team, providing mental health expertise, in addition to community outreach, networking and liaison with community programs.
Essential Functions and Responsibilities of the Job
1.Continuously complies with department quality control/assurance processes.
2.Documents Psychosocial Assessment in accordance to expectations required of the MSW and effectively prioritizes clinical interventions according to acuity level of psychosocial needs.
3.Proactive in avoiding delays in the implementation of the treatment plan to support anticipated discharge date.
4.Collaborates with the healthcare team to provide education, resources and referrals to patient/families and caretakers.
5.Maintains knowledge and ethical practices concerning patient rights, confidentiality, self-determination.
6.Facilitates conferences to enhance communication between providers and the patients and family.
7.Assures that psychosocial and socioeconomic needs are addressed in the treatment and/or discharge plan.
8.Understands and applies appropriate, culturally competent interventions related to death and dying and to decision making issues at the end-of-life.
9.Ability to be at work and on time

Experience
2 years' Acute Medical Care experience required
In lieu of experience, consideration will be made for staff with strong education and associated experiential background.
