We're looking for a Social Worker (MSW) -Bereavement Coordinator to join our team.

You will report directly to the Executive Director or Administrator. You will be responsible for to provide psychosocial support and bereavement needs to the patient and family members.

  • Performs psychosocial assessments, case management, education and ongoing evaluation of patients and families

  • Provides social work expertise regarding changes to the patient's level of care

  • Provides casework and service management of patients requiring information about or monitoring: Family relationships, meaning of illness to the patient and his/her family, cultural and spiritual attitudes in relationship to illness, Long term care planning (including process for placement) and advanced care planning expertise

  • Monitors the safety of the patient environment

  • Providing for the bereavement needs of families through home visits, phone contact, group activities, memorial services, correspondence, and other types of personalized support

About You

  • Master's Degree in Social Work from a CSWE School. Licensure as required by the state in which the hospice is located

  • One year experience as a Social Worker in a healthcare and/or hospice setting is required; three to five years preferred

  • Clinical experience in the areas of life-threatening and chronic illness, grief and loss counseling, individual, family and group therapy. Prefer three years related clinical experience in varied multi-disciplinary settings and one year experience in a healthcare setting

  • Knowledge of terminally ill patients and their families along with understanding of hospice and the psychosocial dynamics of illness, loss, and death

The active duty service members and veterans receiving treatment at the site come from an expansive geographic area. Learn more about this agency Responsibilities The health care, psychosocial problems and needs of this population are complex and require a high degree of clinical oversight, coordination and creative problem-solving. Services are provided at the Beaumont Community Based Outpatient Clinic (CBOC), in the community, virtually and via telework. In addition, incumbent may provide virtual coverage to other CBOC locations within the Michael E. DeBakey VA Medical Center (MEDVAMC) catchment area. Duties include but are not limited to: * Performs specialized treatment of complex physical or mental illness. * Incorporates complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. * Provides outpatient assessment and medication management, supportive therapy, specialized group therapy for veterans with mood disorders, psychotic disorders, and traumatic brain injuries, family treatment and support for all veterans. * Provides mini-mental status exams to all primary care patients being evaluated for early dementia or Alzheimers. * Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services. * Demonstrate the ability to design changes based on empirical findings. * Designs and implements specific programs (i.e. specific therapy modalities) to meet the identified needs for the Rural Mental Health veteran population services at the CBOC Mental Health Clinic. * Coordinates with outside medical facilities to promote continuity of care to include creation of a network of social services providers in the local community to enhance services provided by the VA and fill service gaps identified in the veteran's care and treatment due to limitations in veteran's services. * Functions within the interdisciplinary team to coordinate with medical/mental health staff, residential treatment and care in the community. * Demonstrates the ability to teach and mentor staff and students in the special area of practice and to provide supervision for licensure or specialty certifications. * Offers consultation to colleagues and students on the psychosocial treatment of patients treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. * Worker works collaboratively with the MEDVAMC OEF/OIF Care Management Program Coordinator in providing direct case management services to severely injured (SI) and any identified non-SI veterans assigned to CBOC. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. (2) Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: * Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. * Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. * Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. * Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. * Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
