San Jose Behavioral Health is currently seeking a Social Worker to join our growing team.

$5,000 SIGN ON BONUS ELIGIBLE! SJBH Range = $33.00/hour - $50.00/hour

The Social Worker is responsible for providing both direct and indirect patient care by utilizing counseling skills, social work assessments, high-risk screenings, and age appropriate interventions. The Social Worker manages patients to achieve optimal clinical satisfaction outcomes, facilitates timely discharge planning, and provides patients/families with post-acute options for alternative levels of care.

Conveniently located in the heart of San Jose the hospital is easily accessible to residents in Santa Clara County, San Mateo County, and Alameda County. The Hospital campus is situated amid gently rolling hills and features serene views of beautiful natural surroundings.

Responsibilities

ESSENTIAL FUNCTIONS:

  • Conduct social history assessment of the patient through observation and patient/family interview, obtaining information on the patient's needs, skills, interests and limitations for the development and implementation of the treatment plan.

  • Coordinate and plan programs and activities to meet patient's social and emotional needs.

  • Promote recovery, symptom reduction and the achievement of the highest level of social functioning.

  • Provides crisis management and family sessions to assist the family in understanding the implications and complexities of a life situation and its impact on lifestyle.

  • Assist in the development of effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient.

  • Participate in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge plan.

  • Communicate or aid family members in understanding and supporting the patient.

  • Manage a caseload and assume responsibility for patient chart documentation.

  • Facilitate education, process groups, support groups and referrals as requested.

  • Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.

OTHER FUNCTIONS:

  • Perform other functions and tasks as assigned.

Qualifications

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

  • Master's degree in Social Work from an accredited college or university, including new graduate required.

  • One or more years' experience working with patient population preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

  • Eligible for state licensure as one of the following: Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Certified Addictions Counselor (CAC), Registered Addiction Counselor (RAC), as required by the state in which the facility is operating.

  • CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).

  • First aid may be required based on state or facility.

COMPENSATION

Compensation for roles at San Jose Behavioral Health varies depending on a wide array of factors including but not limited to the specific location, role, skill set, and level of experience. As required by local law, San Jose Behavioral Health provides a reasonable hourly range of compensation for roles that may be hired in California as set forth below.
