Under the supervision of the Department Leader or authorized designee, the Medical Social Worker, in collaboration with the clinical team and medical provider, provides patient and family advocacy, discharge planning coordination and psychosocial intervention for the high risk inpatient. The Social Worker strives to promote patient and family wellness, improved care outcomes and access to appropriate hospital and community resources amount a patient population with complex health needs.
Position Requirements Have a minimum of one (1) year clinical Social Work experience, community based and/or Hospital based required. Verbal ability, assertiveness and diplomacy to deal with position requirements.
Must be able to understand/communicate medical terminology. Produce and discern appropriate
documentation in medical charts and competent to review medical documentation from other
disciplines. Record and keep statistical information. Consistently maintains
professionalism in contacts with patients, families, clinical staff and outside agency
representatives. BCLS required.
