Ecumen Hospice is expanding and is looking for a Social Worker, LSW for our Owatonna location.
Ecumen Hospice is a non-profit, Medicare-certified hospice that has been providing life-honoring care in Minnesota for almost 30 years. We serve those who are experiencing a life-limiting condition and the people that are closest to them, through a comprehensive offering of services which include but are not limited to: physician oversight, nursing, social work, spiritual care, hospice aide assistance, music therapy, massage therapy, volunteer services, pet therapy, and bereavement care.

This is a 30 hour per week position and FT benefit eligible.
This position is responsible for assessing and providing direct social services to residents, patients, family, extended family members and caregivers, so to offer psychosocial, environmental, counseling, and bereavement support.
Essential job responsibilities include:
Assess the psychosocial status of patients and families/caregivers related to the patient's or resident's condition and environment. Carry out evaluations, including family dynamics, caregiver abilities, communication patterns, and abuse and neglect assessments and plans. Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses. Maintains clinical records on all patients referred to social work. Assists family and patient in planning for arrangements, financial, legal, and health care decision responsibilities. Counsel patients, families, extended family and caregivers as needed in relation to stress and other identified coping abilities both prior to and post death. Qualifications
Minimum Required Qualifications:
Bachelor's Degree in social work
Two years of social work experience in a health care setting
Licensed Social Worker in good standing in Minnesota
Current valid driver's license and proof of vehicle insurance
Ability to communicate effectively in both written and verbal formats
Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, Resident's Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and MSDS
Master's Degree in social work
Experience working with seniors
