Position Summary
The MSW is responsible for coordinating care for participants in conjunction with the Interdisciplinary Team, ensuring that participant's emotional and psychosocial needs are met through the duration of their enrollment by providing direct intervention and consultation to other disciplines. The MSW continually assesses participant's needs, effectively communicates, and advocates within a dynamic Interdisciplinary Team. The MSW must be capable of problem solving both independently and in a team setting utilizing full scope of licensure while operating in homes, the community, and day-center.

Required Qualifications
Must possess a Masters degree in social work (MSW) from a program accredited by the Council on Social Work Education (CSWE)
Must have, as a minimum, two years social work experience working directly with individuals
Must have reliable transportation and valid drivers license
Minimum of one year of experience working with the frail elderly, or long term care population

Job Functions
Actively participates in all assessment and care planning of participant's case. Communicates significant changes in participant medical status or psychosocial needs to team members.
Functions as a member of the interdisciplinary team and effectively and promptly communicates participant changes to team members, deeply engages in interdisciplinary team meetings and discussions and the development of the participant plan of care.
Develop a person centered and comprehensive care plan with goals and interventions to actively address participant's needs.
Coordinates 24 hour care delivery. Provides individual family counseling, develops and leads group counseling and activities as indicated.
Coordinates discharge planning with the interdisciplinary team, the hospital and/or long term care facility discharge planners for participants returning home.
Coordinates admissions to long-term care facilities, including skilled nursing facilities, assisted living facilities, or adult foster care homes
Facilitate conversations and complete appropriate documents related to Advance Care Planning including health care wishes and identifying a health care proxy.
Maintains current, professionally-written social work/case management records, including progress notes, care plans and assessments.
Links participants with entitlements through public agencies such as Department of Human Services and Social Security Administration and assists participants with money management and financial eligibility issues.
Refers participants to, and collaborates with, appropriate community agencies and resources, and acts as a liaison and advocate.
Consults with and advises staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
Participates in Quality Improvement Activities.
Documents in the participant's medical record as required by policy
