Social Worker in CASE MANAGEMENT Unit at Providence St. Mary Medical Center
The Licensed Independent Clinical Social Worker (LICSW) independently provides comprehensive biopsychosocial assessment and /or treatment/interventions/advocacy of patients and their support systems. The LICSW incorporates concepts and theories associated with life span development, neurobiology, and behavioral change. The LICSW will collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter-Disciplinary Team (IDT). The LICSW shall perform duties in a manner that respects patient self-determination, that reduces health disparities/barriers to care, and adheres to the NASW code of ethics, Standards of Practice (SOP), and mission.

Required Qualifications:
Master's Degree in Social Work from a CSWE Accredited School of Social Work.
Washington Advanced Social Worker License upon hire. Or
Washington Advanced Social Worker Temporary License upon hire. Or
Washington Clinical Independent Social Worker License upon hire. Or
Washington Clinical Independent Social Worker Temporary License upon hire. Or
Washington Clinical Independent Social Worker Associate License upon hire. Or
Washington Clinical Independent Social Worker Associate Temporary License upon hire. Or
Washington Social Worker Certificate upon hire.
3 years of relevant Social Work experience post MSW.
