Title: Lead Medical Social Worker, Hospice | You would be responsible for assessing the ongoing psychosocial status of the patient and/or family as it relates to the terminal illness and providing intervention to achieve the Interdisciplinary Group (IDG) plan of care goals.

Assist management with ongoing training of hospice social workers and also assist and advise management regarding social work best practices, including documentation and communication processes and expectations. Minimum Requirements:

Masters graduate with a degree in social work from a school accredited by the Council on Social Work Education.

State license of LGSW or above to practice in the State of Minnesota.

Five (5) years of social work experience in a health care setting.
