Social Worker Iii- Group Coverage

Utilizes state and hospital policy guidelines to assist the team in identifying and contacting the appropriate surrogate decision-maker for patients assessed by the physician to lack the capacity for independent decision-making.

Maintains confidentiality and addresses any questions or concerns which patients or families have regarding the patient's rights.

Assists the program director in processing patient/family grievances and in ensuring that patients are treated ethically and respectfully at all times.

Participates actively in treatment team meetings, participates actively in developing the patient's treatment plan, and delivers care based upon the treatment plan.

Participates in the discharge planning process.

May provide direction and guidance to other staff members or may supervise social work interns completing field placement.

Documents in a timely manner all contacts made in relation to patient care in the progress notes utilizing focused documentation.

Must have a LMSW or MSW with plan to be licensed by 2025.
