Social Worker II Case Management FT Days Position Summary

The Social Worker is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for assessing the patient for transition including identifying and assessing patients at risk for re-admission. You will also conduct complex psycho-social assessment and intervention to promote timely and safe discharge and prevent avoidable re-admissions.

This position integrates national standards for case management scope of services including:

  • Transition management: promoting appropriate length of stay, re-admission prevention and patient satisfaction

  • Care coordination by demonstrating quality and efficiency while assuring the appropriate level of care is at the right sequence.

  • Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy

  • Education provided to physicians, patients, families and caregivers

  • Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.

Responsibilities

This individual's responsibility will include the following activities

Complex, psycho-social transition planning assessment and reassessment and intervention

Assistance with adoptions, abuse, and neglect cases, including assessment, intervention and referral as appropriate to local, state and /or federal agencies

Care coordination and implementation or oversight of implementation of the transition plan

Leading and/or facilitating multi-disciplinary patient care conferences including Complex Case Review

Making appropriate referrals to other departments

Communicating with patients and families about the plan of care

Collaborating with physicians, office staff and ancillary departments

Assuring patient education is completed to support post-acute needs

Timely complete and concise documentation in Case Management system,

Maintenance of accurate patient demographic and insurance information

Precepts new staff members and acts as a resource to all staff

Facilitates TEMPO as needed

Participates in department quality improvement initiatives

Other duties as assigned.

THE SOCIAL WORKER II CASE MGMT FT DAYS CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.

EDUCATION

Required: MSW based on license requirements of the state in which the Tenet Hospital operates

Preferred: Accredited Case Manager (ACM)

WORK EXPERIENCE

Preferred: Over 2 years recent acute health care experience. Behavioral health experience

Skills

Required: Leadership; communication and teaching; organizational; interpersonal; team building and collaboration; psychosocial assessment; interviewing; documentation; identification of potential risk situations or potential liability.

Knowledge

Required: Quality improvement process, related health care practices and standards; knowledge and competency in providing care for patients in neonatal, and/or pediatric, and/or adolescent, and/or adult, and/or geriatric age groups; current practices, issues, regulations involved in transitions planning; psychosocial aspects of disease and health care; professional and regulatory standards.

Technology

Required: Basic computer skills, personal technology tools, awareness of current use of technology in related field.

Preferred: Advanced computer skills, technology of the specialty.

Other

Required: Must be able to transport self within the service area as required.

When using own vehicle must have valid Arizona drivers' license and proof of insurance that meets the minimum standards of the state of Arizona. Must be able to take call duty, weekend, holiday and evening/night shift as required.
