Job Title: Social Worker II

Department: Tenant and Community Services

Employment Status: Full-Time; Non-Exempt

Pay Range: $33.00 - $34.66/hour

Location: 34 Turk St, San Francisco, CA 94102

Schedule:

4 days onsite, 1 day remote - to be negotiated with Supervisor

General hours 8:30 am-5:00 pm

Some positions will require one later day 9:30 am

  • 6:00pm

Some positions will require one or two later days 10:00 am-6:30 pm or 10:30 am-7:00 pm

SUMMARY

Under the general direction of the Social Worker/Site Supervisor the Social Worker is responsible for assisting residents living in TNDC properties with housing stabilization & retention by providing counseling and support services, community outreach, advocacy, information, and referrals to existing off-site providers.

Depending on the building or portfolio language proficiency in Cantonese, Mandarin, Russian or Spanish verbal, reading, and writing skills may be required.

TNDC is invested in the professional growth and development of all supporting staff in the Tenant and Community Services Department- Social Work Unit who are eligible to pursue their LCSW, LPCC, and MFT Licenses. This support is demonstrated by entering into agreements with private Clinical Supervisors, providing pre-approved work time for staff to receive private Clinical Supervision, and offering financial resources for many costs associated with licensing requirements. ESSENTIAL DUTIES

  • Manage a caseload of individuals and deliver culturally inclusive support service programs

  • Regularly conduct outreach to assigned tenants, including home visits, to identify their needs and maintain awareness of their overall health status, including medical, mental health, and substance use needs.

  • In partnership with each assigned tenant establish Individualized/Family Oriented Service Plans to outline tenant/family-driven goals, while providing family-focused information and referrals to off-site service providers in the community.

  • Conduct comprehensive Intakes and Needs Assessments with assigned tenants, offering trauma-informed case management and supportive counseling for successful community-based transitions.

  • Provide referrals and linkages to off-site Out of School Time programs

  • Provide Crisis Intervention and conflict resolution

  • Provide housing stabilization, retention and eviction prevention services.

  • Provide services with the Harm Reduction Model.

  • Assist tenants to develop positive exit plans when appropriate and provide tenants with up to 30-days of after-care

  • Collaborate with Property Management for housing retention and eviction prevention, providing necessary follow-up information as required.

  • Serve as liaison/advocate for tenants with Property Management and off-site service providers.

  • Attend weekly meetings with assigned Property Management staff, including on-site Tenant Meetings and monthly off-site community meetings as required.

  • Organize and facilitate community-building activities including but not limited to; produce drops, cultural celebrations, Family large and small group social events, holiday celebrations, and informational workshops.

  • Maintain accurate and up-to-date document of all tenant services using TNDC's CIRCE electronic database

  • Document and monitor tenant intakes and assessments using the SF City and County Clarity ONE System

  • Develop new resources and relationships with outside service providers.

  • Comply with all mandated reporting requirements, including Adult Protective Services & Child Protective Services

  • Assist families with reunification & family sustainability

  • Comply with all HIPAA and PHI policies and procedures

  • Participate in processing all intakes for targeted LOSP and S+C referrals through San Francisco's Coordinated Entry System and in partnership with TNDC's Property Management

  • Carry and respond promptly to cell phone during business hours

  • Other duties as assigned.

Qualifications

MINIMUM QUALIFICATIONS

  • Bachelor's Degree in Social Work or a related field.

  • Two years of experience working with individuals with low-income in a social service setting.

  • Two years of experience providing direct services to individuals with substance use, mental health issues, and/or co-occurring disorders, including linking them to necessary agencies.

  • Demonstrated knowledge of substance use, mental health issues, and their effects on diverse populations

  • DOJ Livescan upon hire

  • TB test upon hire or proof of TB test within the year

PREFERRED QUALIFICATIONS

  • Experience working in a Permanent Supportive Housing setting.

  • Experience providing housing stabilization, housing retention, and eviction prevention services.

  • Experience working with all populations such as seniors aging in place, families and individuals

  • Knowledge of current resources and service/benefits programs in San Francisco and the Greater Bay Area.

  • Bilingual English/Spanish or English/Cantonese or English/Mandarin or Russian/English
