Summary: Delivers various social work services to hospice patients and their families including initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services.

Essential Duties and Responsibilities include the following. Other duties may be assigned to meet business needs.

  • Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of interdisciplinary treatment care plan

  • Conducts ongoing reassessments of patient/family needs and counseling as required

  • Provides short term crisis intervention and individual or family counseling when indicated

  • Provides services, under the direction of a physician

  • Participates as a member of the bereavement team as assigned

  • Observes, assesses and brings to treatment team information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family

  • Assumes an active role of advocate for the patient/family unit

  • Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals

  • Documents comprehensive psychosocial, emotional and spiritual assessment clearly and concisely in a timely manner

  • Records all patient/family visits, telephone contacts and referral actions in the patient record

  • Provides ongoing counseling related to issues of death and dying to the patient and family as needed

  • Attends staff meetings, interdisciplinary team meeting and other meetings as assigned

  • Adheres to all Home Health and Hospice policies

Job Requirements

Education

  • Position requires a baccalaureate degree in psychology, sociology or other field related to social work. Certified Social Worker degree preferred.

Licensure/Certification

  • Service Social Worker (SSW) or Certified Social Worker (CSW) licensure through Utah Department of Occupational and Professional Licensing (DOPL) required.

Experience

  • At least one year of social work experience in a health care setting

Language Skills

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Mathematical Skills

  • Basic math skills are required.

Other Qualifications/Requirements

  • Requires excellent interpersonal communication skills

  • General computer skills

  • Ability to work with limited supervision

Supervisory and Reporting Responsibilities

  • Reports directly to Home Health & Hospice Administrator

Symbii Home Health and Hospice 775 South Main Suite D Logan, Utah 84321 JOB SUMMARY The medical social worker is responsible for the implementation of standards of care for medical social work services. DUTIES & RESPONSIBILITIES 1. Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to the registered nurse. 2. Carries out social evaluations and plans intervention based on evaluation findings. 3. Maintains clinical records on all patients referred to social work. 4. Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs. 5. Provides information to patients or families/caregivers and community agencies. 6. Serves as liaison between patients or families/caregivers and community agencies. 7. Maintains collaborative relationships with Organization personnel to support patient care. 8. Maintains and develops contracts with public and private agencies as resources for patient and organization personnel. 9. Participates in the development of the total plan of care and case conferences as required. 10. Assists physician and other team members in understanding significant social and emotional factors related to health problems. 11. Participates in discharge planning. 12. Graduate of a bachelor's program in social work accredited by the Council on Social Work Education. 2. Minimum of one year's experience in health care. 3. Experience in a home health care preferred. 4. Demonstrates good verbal and written communication, and organization skills. 5. Possesses and maintains current CPR certification. 6. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
