POSITION SUMMARY: In accordance with Agency standards and policies, prepares social histories, provides social casework to clients and families experiencing difficulty in social frameworks. Advocates for referrals to appropriate community resources. Consults and coordinates services with other agency health providers. Assists in development and implementation of health maintenance services, encouraging holistic health care of serviced communities.

ESSENTIAL FUNCTIONS:

  • Conducts psychosocial assessment of the client and family and develops a plan of care and realistic psychosocial goals.

  • Assumes primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluation of the psychosocial components of the patient's plan of care.

  • Prepares social histories to augment service provision, taking into consideration individual and family coping mechanisms, family relationships and dynamics, the meaning of illness to client and family, assessment of the importance and realities of home care provisions and the financial impact of illness on the family.

  • Provides social work therapeutic intervention through counseling, education and bereavement support to client and family to maximize strengths and coping capacity.

  • Promotes client self-determination and self advocacy

  • Assists in negotiating fiscal systems, particularly government entitlements

  • Considers factors related to safety, effectiveness and cost in planning and delivering patient and family care

  • Supervises non-LICSW level social workers when necessary

  • Provides continuous evaluation and monitors services through documentation and consultation.

  • Provides for assistance and advocacy with other community and agency resources as indicated.

  • Participates in agency team meetings and case conferences identifying social problems and assisting in development of health care plan and coordination of services.

  • Consults with staff regarding client care plan concerns; illustrating family dynamics and alternative methods of intervention.

  • Documents in the electronic medical record per agency standards to ensure accurate retrieval of quality indicators

  • Demonstrates knowledge of agency policies, Joint Commission Standards, Medicare Conditions of Participation and state regulations.

  • Provides clear documentation in the EMR (PTCT) so that problems, plans, actions and goals are accurately and clearly stated and changes can be reflected accurately by all members of the team in the plan of care

  • Interprets social resources to agency staff and health services to social agencies.

  • Provides in-service programs to agency staff as requested.

  • Participates in staff education series as appropriate.

  • Participates in development and implementation of new programs and services as requested.

  • Participates in clinical record review for medical social work services.

  • Works flexible schedule, including weekends, to meet patient and family needs

  • Facilitates adult and children's bereavement groups as required.

QUALIFICATIONS:

Current Massachusetts professional LICSW license

Master's Degree in Social Work

Strong customer service skills required
3-5 years experience in community health service or medical social service agency

Hospice and or home care experience strongly preferred

KNOWLEDGE AND ABILITIES:

Ability to read, write and speak English.

Knowledge of social work methods with particular skill in casework and ability to adapt this knowledge to a public health setting.

Ability to communicate effectively with intra and inter-disciplinary professionals

Ability to work with a framework of social work consultations and/or supervision commensurate

with one's experience.

Ability to work in a variety of settings: homes, group homes, skilled nursing facilities and assisted living facilities

Ability to maintain positive attitude despite constant exposure to personal and professional loss

The Drug Court is a specialty court session designed to improve outcomes for individuals with substance use disorders that are involved in the criminal justice system. The Drug Court is overseen by a presiding Justice who, in partnership with other stakeholders, seeks to assist the individual and the community by emphasizing public safety, individual accountability, and recovery. The Drug Court Clinician (DCC) will report directly to the FHS Supervisor of Specialty Courts who reports to the Director of Forensic Court Clinic Services. These forensic services court clinic contracts are managed by the associated Area Forensic Director of the Department of Mental Health. For the Drug Court and participants, as part of their primary duties, the DCC will function as a full member of the Drug Court team and will serve to: * Provide clinical screening, psychosocial assessments and treatment planning for drug court referrals as needed, in conjunction with the probation officer and in accordance with Adult Drug Court operational procedures * Assist in level of care and treatment recommendations for participants as needed * Educate the Drug Court team on clinical perspectives * Assist with care coordination and referrals of participants to treatment * Assist with building linkages with and engaging community based service providers to help enhance the success of Drug Court participants * Help engage participants and their families, in consultation with the Drug Court team, to maximize participant success * Coordinate and partner with probation, and participates when appropriate, on home visits and community contacts * Provide testimony, if required, to the court on the status of each participant * Recommends adjustments to the treatment plan for the participant model in collaboration with the Drug Court team * Support the continuing development of the Drug Court * Work in collaboration with the Drug Court team by providing clinical insights to enhance the development of improvements in the assessment and placement process * Collect and enter data as directed by DMH Forensic Services, and completes any needed documentation on Drug Court participants * Participation in local Drug Court team meetings and in stakeholder meetings in collaboration with the Drug Court team as requested * Assists the Drug Court team in its operations in accordance with Drug Court standards set forth by the Trial Court in conjunction with the Department of Mental Health and the Department of Public Health Bureau of Substance Abuse Services We offer competitive compensation Qualifications The ideal DCC candidate will have * knowledge of evidence based and trauma-informed approaches to substance use disorders, co-occurring mental health disorders and recovery * Experience in treating individuals with these challenges * knowledge of treatment approaches to substance use disorders and ability to support recovery and reduce recidivism in partnership with court personnel and especially in partnership with Probation (who provides supervision of court-ordered conditions) * high quality written and verbal skills * excellent time management skill * demonstrated ability to work independently as well as within a multidisciplinary treatment team * Experience working with court-involved criminal defendants MINIMUM QUALIFICATIONS: Interested candidates must * be licensed as an LICSW (or eligible for licensure within a reasonable time period) * demonstrate exceptional communication, organization, and writing skills * demonstrated experience working with individuals with substance use disorders, preferably over 3-5 years * have experience working with individuals with co-occurring disorders * be able to work collaboratively on a multidisciplinary team * be willing/able to travel * possess a valid driver's license * pass a background check Interested applicants with forensic experience working in criminal justice settings, treatment experience with individuals with substance use disorders, including those with co-occurring mental illness, emergency services, background and knowledge of community and social service agencies are strongly encouraged to apply.
