The Licensed Social Worker (LSW) is to improve the patients psychosocial condition by intervening in those social, emotional, and environmental aspects of the patients life which impact significantly upon his/her overall health status. The LSW achieves this goal by performing home visits with patients to assess the psychosocial needs of the patients.

Assesses each patients social and emotional factors in order to estimate the patients capacity and potential to cope with problems of daily living.
Schedules patient visits as necessary according to patient needs and as per physicians orders.
Helps each patient and his/her family to understand, accept and follow medical recommendations.
Provides services to each patient aimed at restoring the patient to optimal psychological and social adjustment.
Helps each patient resolve social, emotional and/or financial problems which may impede the patients recovery.
Assists each patient and his/her family with personal and environmental difficulties.
Utilizes community resources to assist each patient in resuming life in the community or in learning to live within the limitations of his/her disability or illness.
Assists the physician and other members of the care team in understanding the significant social and emotional factors related to each patients health problems.
Coordinates with all members of each patients care team regarding changes in the patients condition and needs.
Ensures that each patients care plan includes identified psychosocial problems, approaches and goals.
Follows and implements the agencys policies and procedures regarding infection control and safety measures.
Must be currently Licensed in the state of work as a Licensed Master Social Worker
Minimum of one (1) years social work experience in a health care setting
Working knowledge of Medicare/Medicaid home health care benefits, rules and regulations preferred.
A vehicle with current insurance coverage and a valid state drivers license is required.
