The Social Worker (MSW) in Home Health assists patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation. The Social Worker (MSW) will assess, diagnose, and treat patient's mental and social conditions, counsel individuals and|or families, and update case records. The licensed social worker with a master's degree performs these functions under the supervision of the licensed master's degreed social worker.

Additional Details

  • Assists the patient, significant others, physician, and health care team staff to understand significant personal, emotional, environmental, and social factors and difficulties related to the patient's health problems; which interfere with maximizing the benefit of medical services and the plan of care.

  • Contributes as a health care team member to the development of comprehensive, integrated treatment plans for patients.

  • Instructs health care team members on community resources available to assist patients.

  • Plans for continuity of care with hospitals and community agencies.

  • Assesses and treats social and emotional factors related to patient's illness to determine ability to cope with daily living problems.

  • Assists the patient and significant others to understand, accept, follow, and implement medical recommendations.

  • Assists the patient and significant others in utilizing community resources which will help the patient to achieve and maintain optimal functioning.

  • Identifies gaps in community resources and stimulates resource development and/or improvement.

  • Visits patient according to Plan of Treatment; completes a progress note for each visit; and submits progress notes to the agency on an at least weekly basis.

  • Participates in staff conferences and committees as necessary.

  • Provides in-service to agency staff as needed

  • Sends the physician a written summary report on patient's condition at least every 60 days

  • All other duties as assigned


Education Requirements

  • Master's Degree from a school of Social Work accredited by the Council of Social Work Education.

  • One year of social work experience in a healthcare setting.

License Requirements

  • Current CPR certification.

  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

The Master Social Worker (MSW) Case Manager works under the supervision of the Branch Manager to carry out assigned functions related to the social needs of agency clients including consultation, case management, interpretation and supervision of clients and families. The MSW Case Manager ensures that services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures Professional Responsibilities of the Master Social Work (MSW) Case Management: * Assist the client in the ability to understand the significant social and emotional factors related to their health problems. * Assists in the development of a plan of care based on clinical diagnosis, client and physician input that includes medical and social interventions and measurable goals or outcomes. * Arranges to have plans of care implemented by providing linkage to community medical and health resources. * Routinely reviews client progress to determine the effectiveness of services in reaching maximum rehab potential. * ensures use of appropriate medical, social, diagnostic and psychological terminology. * Investigate and take appropriate actions on client or employee concerns/complaints. * Instruct clients, family members and primary care givers as needed to insure the optimal functioning level for each client. * Coordinate agency services by insuring that other agency caregivers are properly orientated and trained to meet the client's needs in accordance with agency policies and procedures. * Insure effective coordination of client home care services through the timely completion of required documentation as well as the timely transfer of pertinent medical data to the client's physician, therapists or other care givers. * Provide input to the Medicare Director regarding the need for formulation or modification of agency policies, procedures, and practices pertaining to client services. * Maintain accurate and comprehensive client data. * Initiate and participate in meetings to develop service plans for agency clients. Requirements * Home Health Care Experience or LTC Experience a plus * Masters Degree in Medical Social Work * Strong assessment and organizational skills * Available for on-call medical questions and/or emergencies from Recover Health Staff * Ability to Multi-task and work under time constraints * Proactive approach to intake management demonstrating leadership skills at all times * Must have a proven, successful leadership background, ability to mentor * Minimum of one year experience in case work with a social or health agency (MSW) Questions: Call Dennis at (262) 782-8292 We are able to set up video chat interviews if you can't make it into the office!
