POSITION SUMMARY:


The purpose of this job is to assist patients and families to access community resources for psychosocial and socio-economic needs.


JOB SPECIFICATIONS:


Required: 1 yr. experience in a Health care setting such as a hospital, rehabilitative center, or clinic, and be knowledgeable in the provision of social services in the home setting. Master#s Degree from an accredited School of Social Work, approved by the Council of Social Work Education. Lic. Master#s Social Worker Basic Life Support/CPR # American Heart Association Counseling skills, including crisis intervention and de-escalation; interpersonal skills; and an understanding of private and public assistance programs Knowledge of: available resources, counseling techniques, and applicable laws and regulations.
