Responsibilities

The social worker in this position functions as a Case Manager for KCVA's Housing and Urban Development /VA Supportive Housing (HUD-VASH) Program. HUD-VASH is an intensive case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans while they are residing in stable housing through HUD's Section 8 voucher system.

The HUD-VASH Program is an essential and critical part of VHA, and vital for ending homelessness among Veterans. Housing Choice Voucher (HCV) Program funding provides rental assistance under a supportive housing program for homeless Veterans authorized by section 8(o)(19) of the United States Housing Act of 1937, 42 U.S.C. 1437f(o)(19). The initiative is known as the HUD-VASH Program, and was authorized pursuant to Division K, Title II, of the Consolidated Appropriations Act, 2008 (Public Law 110-161) ("2008 Appropriation Act") enacted on December 26, 2007 (see proviso (7) under the heading "Tenant-Based Rental Assistance"). The HUD-VASH Program combines HUD HCV rental assistance for homeless Veterans with case management and clinical services provided by the Department of Veterans Affairs (VA) at its medical centers and in the community.

VA Medical Centers must ensure that VA case management, healthcare and other services are made available to homeless Veterans. The HUD-VASH Case Manager is responsible for coordination of intensive case management services for Veterans participating in this program.

The homeless Veteran population has highly complex health and/or mental health problems. The HUD-VASH Case Manager possesses the necessary discerning clinical skills to work effectively with this population. Delivery of these services requires specialized knowledge, capability in resource identification, extensive experience in facilitating access to VA medical/mental health/substance abuse/specialty clinical providers, and community collaboration skills

  • Friday, 8:00 am

  • 4:30 pm, with evenings, weekends, and/or adjusted tours as needed to ensure quality care to veterans.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language: The applicant must be proficient in spoken and written English? Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English.
Education: Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (Must provide copy of transcript(s) with application).
Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. (Must provide copy of license(s) with application). Exception. VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies up to the full performance level. For grade levels above the full performance level, the candidate must be licensed or certified.to the full performance level. Grade Determination:
GS-11 Social Worker: Social Workers at the GS-11 grade level are required to have completed a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the following KSAs:
1.Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational and other diverse backgrounds. This requires knowledge of human development and behavior (physical and psychological), and the differential influences of environment, society and culture.
2.Ability to independently assess the psychosocial functioning and needs of patients and their family members, formulate and implement a recovery treatment plan, and identify a patient's problems, strengths, weaknesses, coping skills, and assistance needed in collaboration with the patient, family, and interdisciplinary treatment team.
3.Knowledge of psychosocial treatment and the ability to independently implement treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. This requires independent judgment and skill in utilizing supportive, problem-solving, crisis intervention, and psychotherapeutic techniques.
4.Knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute/chronic/traumatic illnesses/injuries, effects and side-effects of common medications, and medical terminology.
5.Ability to independently provide diagnosis and counseling/psychotherapy services to individuals, groups, and families. Social worker must be licensed at the advanced practice level.
6.Knowledge of community resources, process for making appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
7.Ability to provide consultation services to other staff members regarding the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new staff and graduate students.
8.Ability to independently evaluate his/her own practice through participation in professional peer review, case conferences, research studies, and other organized means.
9.Knowledge and skill in the use of computer software applications for drafting documents, data management and tracking, with particular expertise in those programs in use by VHA.

Physical Requirements:
The position requires some physical effort.

The incumbent must be able to transport veterans and drive a government automobile in the performance of their official duties. The incumbent must be able to perform community outreach and go into the community including high-homeless areas. The incumbent must be able to adapt to rapid and changing priorities.

A valid driver's license is required, as the incumbent will be working primarily in the community. Must complete annual Employee Health requirements to include a driver's assessment and must pass VA driver's physical.
