Job Description

A professional role in Case Management to develop safe discharge planning for complicated patient and family issues. The role ia a part of a collaborative practice to improve quality through coordination of care.

The collaborative practice includes impacting length of stay, minimizing costs,and ensuring optimum outcomes. Assists with communication of information to area facilities in order to expedite patient transition ot next level of care. Enters data into Case Management Software System.

Assists staff as needed, and assists with other assigned duties, tasks, and projects. Ability to effectively communicate with all patients, families, hospital staff and physicians.Possess the ability to be detailed oriented.Ability to stand,sit, and walk throughout the hospital setting.Maintain current clinical knowledge,Utilize social work processes,skilled in psychosocial assessment, skilled in negotiation and problem solving, demonstrates professional, assertive communication skills,effectively documents discharge plans in medical record and reviews data for patterns and resource consumption. Technical knowledge of automated support system and maintenance of same.

Knowledge of basic record keeping and filing systems. Demonstrates skill at professional business written and verbal communications. Ability to prepare reports and follow up with minimal supervision.

Self-motivated. Demonstrates appropriate application of computer, fax machine, telephone and copy machine. Must be able to follow directions and to perform work according to department standards independently. Must be able to function effectively under stressful conditions.

Preferred Education: Licensed Master's in Social Work for NICU, ICU

and ED

Required Experience: Hospital experience or health care setting

Experience: 1-2 years of hospital Case Management/ Social Work/ Discharge Planning/ and/or Administrative.

Preferred Experience: Acute Care Social Work experience

License/Certification: Licensed Social Worker required
