Facilitates timely and appropriate discharges from the hospital;identifies inappropriate or untimely services; identifies opportunities forimprovements in patient care; assists patients/families in obtaining help inrelation to disability, illness, financial hardship or other problems;functions as a liaison with insurance/managed care/utilization review companiesand provides documentation of the above per regulatory requirements andhospital policy.
ESSENTIAL FUNCTIONS(including, but not limited to):
Collaborateswith RN Case Managers in the plan of care of patients and families within theacute setting and along the healthcare continuum.
Assiststhe RN Case Manager with updates from insurance providers on dischargeassistance that can be provided for the patient/family.
Actsas a liaison through effective and professional communications between the CaseManagers, hospital staff, outside agencies and patient/family.
Assiststhe RN Case Manager in the plan of care to ensure timely and cost-effective useof resources.
Assistswith Baker Act placements by updating and communicating needed information tooutside facilities.
Assistswith appropriate after care needs (i.e. HHC, SNF, hospice, etc).
Assistsin finding community resources for unfunded patients.
