Job Summary

The Social Worker/Counselor II leads the planning and coordination of patient care. Responsible for providing social services for patients and families of inpatient behavioral health as part of a multidisciplinary team with emphasis on group and individual therapy.

Works closely with psychiatrists, as an active member of the Treatment Team by attending Care Plan meetings. Responsible for psychotherapy group, wellness groups, psychosocial assessments and facilitating individual, marital and/or family therapy. Develops, implements, reviews and updates treatment plans as well as case management and discharge planning.

Ensures assessments are completed within regulatory timeframe. Has ability to perform all requirements of the social worker/ counselor I.

Leads weekly clinical supervision meetings as assigned. Designs and implements specialized therapeutic groups for patients and families in all program areas, including education, support services and community resources.

Demonstrated ability by education or training to treat people with mental health co-occurring diagnosis.

Must have considerable knowledge of therapeutic interventions and age-specific needs of both geriatric and adult populations likely to be served.

Knowledge of the physiological, sociological, and psychological implications for the disease processes experienced by the population served.

Ability to evaluate patients and interpret data

Ability to plan, develop and facilitate psychotherapy groups.

Ability to evaluate client progress and to present evaluative reports in interdisciplinary team meetings, facilitation of treatment team meetings, Must have clinical competence to provide therapeutic services to the mentally ill demonstrating empathy, compassion, and strong interpersonal skills

Ability to use critical thinking and clinical reasoning skills including but not limited to analysis problem solving, and decision-making

Ability to exercise self-control in potentially volatile situations such as being verbally or physically confronted in a threatening or aggressive manner. Must be able to set and maintain therapeutic distance with patients; maintain therapeutic boundaries

Ability to efficiently manage time, priorities and resources

Possesses basic computer skills; demonstrated proficiency in Microsoft Office and web-based applications

Requires full range of body motion including handling and lifting patient, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extended periods of time.

Frequently lifts and carries items weighing up to 100 pounds (with assistance). Requires corrected vision and hearing to normal range. Requires working under stressful conditions and irregular hours. Exposure to communicable diseases and/or body fluids.

Other duties as assigned by manager or director.

Qualifications - Minimum

Certificates/License/ Registrations:

Current unencumbered applicable state license in social work or counseling. Certified Group Psychotherapy (CGP) within 3 years of hire.

Education and/or Experience:

Minimum Master's degree in social work/counseling. Licensed Independent Social Worker or Licensed Practical Clinical Counselor preferred.

Licensed Independent Chemical Dependence Counselor optional. 3-5 years' experience providing inpatient and outpatient treatment to individuals with co-occurring mental health disorders and substance diagnosis. This includes individual, group therapies. Experience in working in a psychiatric setting highly preferred. Basic Life Support Certification: required within 30 days of hire.

Special physical requirements: sits and stands throughout the day. Physical management of patients in crisis situation may be necessary.

Required Job Specific Training: De-Escalation training within 90 days of hire; Group Psychotherapy Training

The Social Worker/Counselor II must provide individual and group treatment in both inpatient and outpatient setting, provide assistance with discharge planning, assessments and psychoeducational groups, and perform Intake/Clinical Assessments in the ED, for scheduled assessments/walk-ins, or on the medical floors. The Social Worker/Counselor II documents in medical record in accordance with accepted policy in a timely manner. A licensed independent chemical dependency counselor may be asked to provide supervision to staff eligible and pursuing this licensure.

The Social Worker/Counselor II Adheres to the standards and policies of the Corporate Responsibility Program, including the duty to comply with applicable laws and regulations, and reports to designated Manager (or employer hotline) any suspected unethical, fraudulent, or unlawful acts or policies. He or she will demonstrate ethical, sound professional practices and personal accountability and act in alignment with the mission and core values of Catholic Health Partners.

The Social Worker/Counselor II participates in administrative meetings and committees (staff meetings, clinical supervision, quality improvement, patient satisfaction, employee engagement, etc.) to exchange information that relates to program updates, identify program barriers and anticipate future program needs in accordance with appropriate accreditation standards and within facility policies. He or she participates in the supervision of clinical interns or students and/or volunteers, utilizing orientation, observation, record reviews, feedback and evaluations as required.

The Social Worker/Counselor II participates in the safe management of the milieu, intervening effectively with patients who behave inappropriately or who are having coping difficulty, utilizing effective non-abusive crisis prevention/intervention techniques and treats patients and families with empathy and respect.

The Social Worker/Counselor II maintains effective working relationships with team members, (requires both flexibility and setting limits) and performs work in accordance with the facility standards and policies regarding risk management, safety, infection control, etc. He or she will performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others. The Social Worker/Counselor II Develops annual personalized professional goals in conjunction with manager/Director, and seeks and participates in learning opportunities that meet personal and departmental goals.
