JOB SUMMARY: Healthy Harford, Inc., founded in 1993, is the Healthy Communities Initiative of Harford County and doing business as Healthy Cecil.# ##The mission of Healthy Harford/Healthy Cecil is to inspire and empower healthy people, healthy families and healthy communities in mind, body and spirit.# The focus is to improve health and wellness in Harford and Cecil Counties by promoting healthy lifestyles, building community partnerships and providing care coordination.# # The Social Worker is a member of the Healthy Harford/Cecil Community-Based Care Team.# Through an integrated approach, the social worker will be responsible for completing intake assessments, assisting with treatment planning, monitoring client progress, facilitating/conducting client and family meetings/home visits, and coordinating and implementing treatment referrals.

This position will provide direct clinical services to clients through psychotherapy utilizing brief therapy models incorporating crisis stabilization and cognitive behavioral techniques.# The social work services will include community outreach, social support, care transition and community service linkages.# The position requires accountability for planning, and organizing community-based services that support clients# self-management, adherence to treatment plans, and reduction of unnecessary re-hospitalization. Education


Training:


MSW from a school of social work accredited by the Council of Social Work Education required.# Maryland licensure at the LGSW, LGPC, LCSW, LCPC, or LCSW-C required.# Training in aggressive patient management required. Work Orientation


Experience:


Experience working with individuals with acute, chronic, and social needs required.# Acute care or behavioral health social work experience preferred.# Understanding of insurance eligibility and coverage helpful. Skills

a) demonstrate knowledge/skills of clinical social work; b) demonstrate effective critical thinking; c) communicate and collaborate effectively with both internal and external clients (colleagues, medical staff, liaisons, and client/family) in writing and verbally; d) develop and implement psycho-social assessments, continuing care plans and aftercare plans; e) coordinate services for client/family with community providers including knowledge of community resources, Medicare/Medicaid and commercial insurances; f) prioritize work assignments and manage time effectively to complete duties; #g) work effectively and calmly in a fast paced environment; h) make independent decisions consistent with current policies, procedures, health care and ethical standards; i) be available to adapt hours to meet the needs of all clients; j) demonstrate an understanding and sensitivity to serving a culturally diverse population while respecting patient confidentially; and k) must have reliable transportation.

Performs individual and group counseling for patients/family with a cancer diagnosis. Provides crisis intervention and emotional support to cancer patients and their families. interacts with patient/family populations from all cultural ethnic and socio-economic backgrounds. Supervises the work of the volunteer staff to coordinate supportive services offered through the oncology Supportive Care Program. Proactively serves as resource to physicians and team members for the psychosocial management of a patient with a cancer diagnosis. Is an active member of the health care team and serves as a liaison/patient advocate between the patient, family and medical providers. Is an active participant in the Oncology Service Line committees and performance improvements activities.Qualifications: Masters Degree in Psychology, Counseling or Social Work. Current licensure or license eligible as a family therapist or professional counselor through the Maryland Board of Professional Counselors and Therapists preferred.
