At Collective Health, Care Navigation plays a crucial role in helping people get the right support at the right time across a wide range of complex health needs. Care Navigation is a comprehensive care management program made up of our team of pharmacists, social workers, nurses, dietitians, and care coordinators who support people dealing with chronic or serious illnesses by guiding them on their journey to help them understand and navigate the disjointed U.S. healthcare system. In joining our Care Navigation team, you'll have the important role of helping our members achieve better health at a lower cost. You will report to the Senior Manager of Care Navigation.
What you'll do:
Leading outreach efforts to high-risk individuals with complex medical conditions through telephonic or electronic outreach
Completing telephonic social work assessments and care plans for patients with behavioral, social, financial and environment problems
Educating and empowering members using decision support through multiple modalities including telephone and email to help members achieve their health goals
Identifying and discussing current barriers related to member's current condition, and provide interventions to address these barriers
Providing emotional support, assisting with benefits and behavioral health navigation
Coordinating a range of services, including community resources and health benefit resources
Utilizing motivational interviewing to assist members to become self sufficient in their care planning
Participating in clinical interdisciplinary rounds
Working collaboratively on co-managed cases with nurses, pharmacists, and dietitians
Working cross functionally with other teams to assist with benefits and claims questions
Participating in program case rounds and providing input for process improvement opportunities and resources for team
Will be required to take call as scheduled
Required Travel: 2 days per quarter with client-facing and member-facing presentation and interaction

To be successful in this role, you'll need:
You have an MSW or LCSW from an accredited school of Social Work
You do not require hours of supervision for licensure. Collective Health will not be able to provide supervision for clinical hours. Collective Health will not provide supervision or experience with Clinical Psychosocial Diagnosis, Assessment, Treatment, Individual or Group Psychotherapy.
Experience in case management; experience at a payer is a plus
Bilingual Preferred (English/Spanish)
You are passionate, empathetic, and excited to help people
You are independent, tenacious, and capable of identifying and implementing best practices
You are capable of thinking creatively to implement care navigation solutions that challenge status quo
You are driven, able to carry a caseload and prioritize to complete daily tasks and outreach
You are tech savvy; able to talk, type, and navigate an electronic chart with ease
Proficient in Google suite, such as Google Sheets, Docs, etc.
You are excited to work in a telephonic social work role
You are okay with a role that does not involve providing individual therapy
You are a problem solver using best practices to achieve goals
You are comfortable with ambiguity and open to collaborating with people across different teams
Additional Case Management Certification is a plus
Discharge planning, complex care coordination, behavioral health, and/or home health, oncology, behavioral health, autism, pediatrics, and/or maternity experience a plus

This is a hybrid position based out of our Plano, TX office, with the expectation of being in office at least three weekdays per week. #LI-hybrid
The actual pay rate offered within the range will depend on factors including geographic location, qualifications, experience, and internal equity. In addition to the salary, you will be eligible for stock options and benefits like health insurance, 401k, and paid time off.
