Promotes achievement of patient and family's optimal level of physical and social functioning. Facilitates and coordinates development of a discharge plan of care for high risk patient populations. Works in care management and assists patients and families in understanding patient conditions and treatments. Help patients and family members develop coping strategies, assess patient needs, assist in the development of treatment plans and provide emotional and mental assessments on the patient. Bachelor's degree in Social Work from accredited school is required. Master's degree preferred. One to two years experience in counseling and provision of other direct social work services. Three months to one year experience in counseling and provision of direct social work services in a health care setting. Excellent communication and interpersonal skills, negotiation, advocacy skills with patients, families, staff and community agency personnel. Computer skills also required. Work performed primarily in office setting with some work performed on patient care units. Requires constant hearing, speaking, vision and repetitive hand movement. Requires attention to detail despite frequent interruptions. Under certain conditions, may be exposed to blood and/or body fluids or potentially infective materials.
