The Long Term Services and Supports (LTSS) Service Area of the Human Services and Public Health Department (HSPHD) is seeking a Social Worker to complete initial assessments. In this position you will complete assessments utilizing the MnCHOICES assessment tool and will be a member of an interdisciplinary team of Public Health Nurses and Social Workers. The Long Term Services and Supports assessments provide referral and planning, are person-centered, strength and needs based, and serve to inform recipient of options, determine eligibility, level of care and service needs. The team administers Medicaid waiver and state programs that in addition to serving frail seniors, serves individuals with brain injury, mental health, complex medical, developmental, and physical disabilities.

Location and hours:

This position is hybrid and will be performed both on-site at various Hennepin County locations and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:

  • Conduct person-centered comprehensive social/health/medical/psych assessments.

  • Create person-centered support plans utilizing formal services, community resources, and informal supports, making referrals as necessary.

  • Determine applicants' risk of institutional care and their level of care according to state and federal criteria.

  • Assist the applicant in identifying relevant goals along with specific supports that will enhance a person's ability to meet goals.

  • Provide eligible applicants Waivers and PCA services according to availability.

  • Document data and case activities in State and County computer-based programs.

Need to have:

  • One of the following:

  • Bachelor's degree or higher in social work, psychology, sociology, human services, or an equivalent field.

  • Licensure as a social worker by the Minnesota Board of Social Work

A valid driver's license and daily access to reliable transportation to travel primarily within/outside of the metro area to visit clients in their residential setting. Nice to have:

  • Experience with:

  • Providing home and community-based services.

  • Conducting person-centered comprehensive assessments.

  • Documenting data and case activities in state and county computer-based programs.

Knowledge of:

  • Medicaid Waiver programs and MA Home Care services.

  • Payer sources for home and community-based services and existing community-based services.

Ability to demonstrate interpersonal, organizational, and person-centered skills.
