Performs a variety of services with all patient populations including psychosocial needs assessments patient / family education regarding community state and federal resources. Provides counseling to patients and families about impact of hospitalization and illness on well being.

Requirements: Minimum MSW or equivalent 1-3 years experience Major

Responsibilities:
Assesses all patients referred for social work case management services. Referrals are received from physician orders; nursing orders; multidisciplinary rounds; patient / family requests and community agencies.

Prescreening for high risk patients is routinely performed via census review and multidisciplinary rounds. Performs needs assessments; prioritizes patients# needs / choices; and collaborates with the interdisciplinary team; patient / pertinent other; and community resources in order to fully develop/organize treatment plans with optimal outcomes. Coordinates plans with patient / family; members of the health care team utilizing best practice approaches.

Performs on-going evaluation / readjustment of plans for efficacy; quality; cost effectiveness; and goals / outcomes. Serves as patient liaison / advocate between care providers; community agencies; and insurance payers. Advocates for services and funding necessary to meet established outcomes.

Maintains working knowledge of payer requirements. Provides supportive counseling and crisis intervention and advocacy to patients and families. Works in close collaboration with nurse case manager on all cases and; in particular; cases involving complex biopsychosocial / legal / financial issues to establish and review plan of care.

Prepares and maintains required documentation for each case. Complies with State and Federal mandates for documentation and reporting.

This social worker will collaborate with employees and dependents to assess, develop, and implement care plans that will lead to improved self-management and wellness including identification and reduction of barriers to care plan adherence, optimizing health plan benefits, education, and referral to community education programs as needed.# # Requirements: Master#s in Social Work Valid Driver#s License Case management with payer focused care preferred Proficient with or willing to learn Excel Strong ability to develop rapport # Responsibilities: Works as the liaison between the actively enrolled self-insured members of EHR and their healthcare team.# Advocates for and facilitates resolution of patient questions and concerns.# Provides or arranges patient education regarding specific health care skills, disease concepts, and health plan benefits. Working with both pediatric and adult participants, conducts comprehensive clinical assessments that include ageâ€'specific, medical, behavioral, pharmacy, and social needs of each patient. Shares this information with the healthcare team and the patient/caregiver to develop and monitor adherence to an individualized care plan.# Documents within the electronic medical record.# Works toward continuously improving the process to create efficient, meaningful, and effective care management and care plans. Partners with patients and families with the goal of self-management support including: a. Sets short and longâ€'term goals for selfâ€'management of chronic disease to include acute exacerbation management b. Performs reassessment in patient progress toward goals, assesses barriers, and alters plan of care as appropriate. May assist with identifying and scheduling patients for gap closure or post transitional care needs. This may include preventative or chronic care visits or inpatient/post-acute care follow up. Conducts assessment and implementation of services and support identified social determinants of health. When appropriate, recommends use of therapeutic interventions to support the individual(s) in collaboration with the Nurse Care Coordinator # EHR to identify and reduce barriers to care and optimize patient activation. Partners with data analytics and the health plan to utilize a population based registry and associated electronic portals, monitor and report on quality, and improved patient outcomes. Develops standard work around use of registries and the role of various team members in registry management. These tools will be used to proactively identify the target population. Participates in quality improvement projects aimed to improve patientâ€'population outcomes and associated processes both within Core Physicians and across the healthcare system/affiliates. Facilitates engagement through telephonic or face to face visits. Meets with patients and/or families, individually or co-visits with nursing or other clinical team members. The aim of this interaction is to improve adherence to goals and related behavior modification.## Helps design and implement educational and engagement material. Works across the system and collaborates with leadership to utilize current programming and/or design.# Implements new programming that supports the population and their stated goals.
