Summary of Position:
The Hospital Social Worker provides social service interventions and assists with discharge planning for patients in the Medical, Orthopedic unit(s), ICU, OB, and the emergency department utilizing established standards and practice, ensuring safe and effective intervention. This position may provide care coordination across the continuum. Intervention is based on the ages of the population served; Newborn through geriatric
Qualifications
Education:
Minimum Education:
● Masters in Social Work from a program accredited by the Council on Social Work Education required.
Experience:
● Previous experience as a clinical social worker in a medical setting (hospital, outpatient clinic, etc.) preferred
● Previous computer experience and database management preferred.
Knowledge/Skills/Abilities:
● Assessment and referral skills mandatory
● Knowledge of behavioral health diagnosis and treatment preferred
● Knowledge of addiction and treatment preferred
● Knowledge of Medicare and Medi-CAL preferred
Certifications/Licensure:
● LCSW preferred

Essential Functions
1.Acts as professional liaison with Medical Staff
2.Participates in hospital & Medical Staff committees as required. Represents hospital in professional & civic organizations.
3.Has extensive knowledge of all professional and regulatory standards that apply to all departmental activities.
4.Performs a social service assessment on all identified patients within organizational timeframes.
5.Gathers and integrates data to determine social service and discharge needs of patient.

Mental Status
6.Determine presence of any barriers to a safe and effective transition from the acute inpatient to a lower level of care.
7.In collaboration with the multidisciplinary team, develops a plan based on the assessed needs of the patient.
8.Conducts multidisciplinary plan of care conferences.
9.Collaborates with other disciplines and community agencies
Communicates family issues and concerns to care team
Provides education and support to assist patients and families to understand, accept and follow through with medical recommendations. As evidenced by MR documentation
Utilizes all internal and external resources and referrals to assist in meeting the unique needs of the patient.
Expert on child, elder, dependant adult abuse and domestic violence. Regulations, reporting requirements and social service interventions. Is an organizational resource in the identification and reporting of suspected cases of abuse.
