We Are Hiring:
Schedule is 7-5 or 4 ten hour shifts a week. One Saturday every 10-12 weeks required.
What You Will Do:
Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
Create plan for care across the continuum, integrating patient/family preferences and values.
Monitor patient care through assessments, evaluations and/or patient records.
Advocate for resources and removal of barriers.
Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
What You Will Need:
Licensure / Certification / Registration:
One or more of the following:
Clinical Counselor obtained prior to hire date or job transfer date.
Counselor obtained prior to hire date or job transfer date.
Social Worker credentialed from the Alabama Board of Social Work Examiners obtained within 3 Months (90 days) of hire date or job transfer date.
Social Worker obtained prior to hire date or job transfer date.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Would prefer Hospital CM experience but not required.
