JOB REQUIREMENTS: Master's Degree in Social Work from an accredited program. BLS Certification.

Minimum of 1 year full time experience in Care Management as a Social Worker in an acute care setting within the last year. Proficient in supporting patients' access to post acute care services and appropriate utilization of resources. Collaboration with post-acute care providers to achieve optimal health and ensure patient needs are met in the community.

Must show competency with assessment, discharge planning/intervention, implementation, care coordination, monitoring and evaluation of the plan of care. Proficient in managing a caseload of high risk, complex needs and/or catastrophic patient(s).
